Determining The CAUSE OF MICROSCOPIC HEMATURIA

    It’s not unusual to be taken by surprise when you see microscopic hematuria on urine dip results. The patients who end up with this as part of their presentation are often asymptomatic, and may even be giving you a urine sample for another reason - which can make chasing down the cause even more of a mystery.
    If you don’t know where to start when it comes to determining the cause of microscopic hematuria, you’re not alone. In this video, we will cover a high-level approach to investigating a finding of microscopic hematuria. We will go over the workup for microscopic hematuria, and lots more:
    ✅ What is the difference between microscopic and macroscopic hematuria?
    ✅ When a urine dip is enough, and when you need to send the sample out
    ✅ What white blood cells and protein in the sample can tell you
    ✅ Essential history and symptom questions to ask
    ✅ When to repeat the urine dip at another time
    Having a simple framework for approaching the sometimes mysterious microscopic hematuria will increase your confidence and help you to take the best care of your patients. Getting to the root cause of microscopic hematuria is another critical part of taking great care of the patients you work with.

    Hi!
    I’ve only taken about one health related course in university, so breaking down my lab results showing trace blood in the urine was a bit daunting for me 😅.
    It’s been a week or so for my PCP to get back to me on the results, so this has helped me a lot! Thanks for the breakdown :)

    Hi, great vid, had a UTI that cleared but still find a +(1) blood in urinalysis..a urine culture with no growth (bacteria,parasites,negative) .no symptoms,have had sti screening,high viginal swab plus a Pap,all have turned negative... what could be the issue?

      Overall it correlates with the amount of blood present in the sample but it's dependent on a few factors!       You can slow down the video speed on your own on here. Under settings, then playback speed pick one. I picked 0.75 playback speed.

    What about varicocele?

      You know I haven't seen that myself and I don't see anything on my resources that suggest that varicocele can cause hematuria. That would be something to look into futher!

    Why would a dipstick always be positive for microscopic blood, but went send out for analysis for presence of blood under microscope it’s always negative?

      There could be MANY causes but should definitely be investigated. As a provider you should determine if additional work up or specialist referral is needed to determine the cause.

    GREAT VIDEO! THE TEST STRIPS CAN BE WRONG! AND MANY DOCTORS DO NOT SEND THEM OUT BUT START TREATING YOU WITHOUT KNOWING WHAT THE REAL LABS REVEAL!

      No neurological causes that I'm familiar with - typically renal, urologic, or "mimics" from structures in the area (perineum, anus, etc) or medications/foods/false +

    NP here, working in Urology, unclear why you make the distinction of Urological parts vs Kidney/Nephrology? Better to just describe as upper vs lower GU tract. But the terminology of "Urological" structure is misleading, the kidneys are Urological by definition.

      I think she was describing Nephrology and Urology to hint or give guidance to what service to refer the patient to depending on what the labs show and what symptoms the patient has. I understand that kidney is part of the the urological system and some may get confused but I totally understood what she was saying.
