    Children with medical complexity (CMC) have complex, chronic diseases involving multiple organ systems, and are often technology-dependent and frequently have significant disabilities. Families of CMC require intensive care coordination to meet their diverse needs. Care coordination is a patient and family-centered, assessment-driven, team-based activity designed to meet the needs of children and youth while enhancing the caregiving capabilities of families. Family goals should be incorporated as key elements into shared care plans.
