I love the fact that the 2 ladies in the back are having a great time

I watched this a few times before I realized that the two Japanese Ladies sitting behind them are the funniest thing about it. Them trying to be polite and not laugh at the awkwardness of the situation makes me smile every time.

Those subtitles aren't the real translation; I think the video creator is just experimenting with subtitles. I can't understand the doctor much, but this is the waiting room scene:
Patient: Nihon ni nan nen irun desu ka? (How many years have you been in Japan?)
    Bill Murray: In deska?
Patient: Irun desu ka? (repeating the end of the sentence)
Ehh so... Eigo ga wakaranai ya ra na... (Ahh, I don't know English...)
Nihon ni... (In Japan...) Japan. Appu. Japan Appu. ("Appu" is a Japanese pronunciation of the English word "up." I think she's trying to say "In Japan." "Up" and "on" are the same word in Japanese so I think she's mistakenly saying "On Japan.")
Japan up... nan nen? (How many years?)
Ahhhh... (She gestures a rotation of the earth, punctuated by the word "sou" which roughly translates to "like that.")
I wonder if this is a real encounter, not scripted. You can see the women in the background laughing, and the patient's reaction to Bill Murray's "song" seems genuine. Foreigners in Japan often get questions like this from strangers so I wouldn't be surprised if this happened while they were shooting the movie and they kept it.

LOL Broooo these subs are not right at all hahahaha - the old lady first asked him if he was done, then she said, how many years have you been in Japan? Nihon-ni, nan-nen irundesuka? And Bob was saying like "N-desuka" and that set off those ladies behind him because that was funny as hell. Then the old lady was trying to explain a year to him with that circle-drawing - like, how many moons have you been here etc.
