You try looking out for 300+ passengers when there are only 11 crew, half of them who are probably trying to get everyone desperately out of a burning aircraft. They can only do their best to try and get everyone out of the aircraft as fast and safe as possible. They are still heroes for saving people's lives, but they can't monitor every little minute detail of whose been injured and who hasn't amongst 300+ people when there's only 5-6 of you.
I live in Japan and all the major Japanese reported it. A quick check on the Internet also shows many articles confirming that Facebook's COO Sheryl Sandberg intended to be on that flight.

its wied how they didnt mention the 16 year old girl that was killed, as she was pulled "to safety" laying on the ground, next to the plane, 2 firetrucks drove over her and killed her :(

The girl was covered with firefighting foam, hence why no one saw her. Oh, and it was later determined she was already dead when run over.
