Actually very easy but I could not take the content from your psd to put on the glass rectangle.. still I dont know hot to do this because o the layers.
Followed this to a t and repeated the process multiple times but i can move the blur around like you do in this video it just moves the blured image behing it almost acting like a window to the blurred image, move it far enough you can see the black from the rectangle. Anyone else having this problem?

When using the move tool (v) make sure to uncheck "Auto - Select" and "Show transform Controls"
