Am a Radiographer, when I saw all this, my impression was about injury to the spleen. Possibly the sonographer was not able to assess thoroughly for acute splenic injury, acute hemorrhage in the spleen/acute splenic injury can be quite challenging to confirm. Since the hemorrhage was still concealed within the spleen, made it more challenging. So possibly serial scans had to be done with close monitoring of all other aspects. He could have benefited from emergency surgery. So probably, there was weakness also there in decision making.
To b honest, there was weakness in managing the emergency. But I also realise, the medics try to follow certain protocols set by the institution to keep a balance.

Did you not check his VITAL SIGNS, (his ABCDE), A: Alert, B: Breathing, C: Circulatory; D: Disability and E: Exposures. His circulation wasn’t good, lowering his blood pressure, and depriving him of oxygen to vital organs, like brain, heart, etc; causing sudden death. Bleeding internally also cause loss of fluids, could have caused hypovolemia shock. Bambi kitalo, naye yadde omuntu agudde kigwo, check their vital signs. You can also check circulatory through capillary re-fill, 5 and 2 seconds. If that is normal, then check pulse rate, to check if there is any irregular heart beats, check their blood pressures. Then keep him resting. That standing affected oxygen concentration, and dropped into unconsciousness, and probably into cardiac arrest ! 😢 Mulabika temwamussa na ku ECG to monitor heart activities !
