I didn't see any mention of the exposed forks at the 4:30 mark? People tend to forget about having a short pallet on their forks. It will slide back resulting in exposed forks, I've seen it happy many times before. That in turn can cause serious bodily injury and/or damage to property...

when I worked at Deseret Industries as an item picker I saw tons of violations these incidents got six members of my crew fired. 7:07 loose load one of my coworkers was speeding and one of the crates being transported tipped over I had to clean it up. another incident not covered in this video FORKLIFTS ARE NOT LADDERS! if you want to use a forklift to get onto a rack or reach a high spot use a cage don't stand on the forks this is dangerous and illegal.

Also, as an additional point; Forklifts should NEVER be used to lift a person if possible- Manlifts and Scissor-lifts are specially designed for this purpose, and are much safer. If it is absolutely necessary to use a Forklift to lift a person, you should not let them stand on the forks, a pallet, a load, or anything other than an OSHA-approved man-lift attachment.

The forklift driver at 9:20 didn't look backwards when reversing after stacking, that's bad driving.

Those demos of how to lift the load from the racking are bad. I would have failed my test if I had lifted like that.
