The lady in black speaks too fast, hence swallows certain syllable sounds, thereby mispronouncing words. On the contrary, the one playing the role of the nurse, has a moderate and appropriate rate of speech.
She must be the mentor... And playing the role as a nurse candidate
That's how it's supposed to be ,so in a real scenario would you tell your patient that he/she is speaking fast and swallowing words and henceforth its tricky or hard to assist them, will you, huh ?

In actual setting, the doctor/ nurse may politely interrupt and request the patient to slow down, in order, for the recipient of the information, to clearly comprehend what is being communicated. On the contrary, responding hastily without fully understanding the patient, will result in misinformation or trigger confusion and dissatisfaction with the health professional.
In all humility, let me bring to your notice, I'm one of the pioneers to be certified as an OET trainer, PAN India. In fact, a significant numbers of teachers have been receiving comprehensive coaching under my guidance, since 2004 ( When the OET format was newly introduced in our country.)
