      Gus never noticed Walt .
Gus actually sharted before he made it to the car. He refused to use hospital bathrooms as they are too filthy for his suit... But when we was almost at the car, he stopped and made sure there were no witnesses in the car park, then he sharted. Upon knowing this (guards obviously knew too) they immediately went to secure location where Gus has a fresh set of undies.

For those wondering what happened here: Gus realized that things were not making sense. Jesse had him come to the hospital. He then told him that a sick child had been poisoned and they don't know how. Who would've poisoned the kid and why? He knows it wasn't him. Then he realized that his car had been left unguarded.
