I learned a lot from this video. I am 44 yo male, have been having same exact symptoms and flow rate of 7 ml/s too. Cystoscopy showed “high bladder neck”. I’ve tried also alfuzosin and mirabegron but I don’t like the side effects and didn’t take it long enough to get a good result maybe. I’ve been advised to do a BNI but I was afraid of doing that. So either another cystoscopy (since it’s been over a year since last one) or a urodynamics study now. I was interested when you said MRI of spine, because I have incidentally had some low back pain off and on from heavy lifting and was thinking of getting an MRI. Thanks
Very similar symptoms and failing alpha blockers and treatments. I first encountered the issues at 47, had 3 MRIs done yet inconclusive... Prostate was 28cc and slight BPH was mentioned. Was told also I had "high bladder neck" but then no recommendations nor explanations on what could have been the cause and guidelines to prevent re-occurrence. Eventually had Rezum done after 18months CIC, which improved matters somehow yet with an episode of urine retention after one week, then retrograde ejaculation improving gradually 4 months down. PVR still at 100ml+ depending on the day... Still I am amazed most data available is questionnaires and statistics yet no treatments nor explanations... Seems like urology is still at the occult science stage 🙄🙄🙄.
I was advise to undergo a bladder neck incision but then heard about UroLift and Rezum as alternatives to preserve ejaculation. But, all the doctors have told me that because my issue is with the bladder neck, then methods such as Urolift or Rezum would NOT preserve ejaculation because the bladder neck is the one being treated, as opposed to the prostate itself, which is what Urolift & Rezum were designed for. Does this sound right? I'm intrigued by the conversation towards end of the video about how messing with the bladder neck is not the primary factor in causing anejaculation.
@@monusolanki8584 I haven't done anything yet. Urolift has been discarded because my prostate is small, iTind doesn't seem advisable per various doctors, so I'm thinking Rezum in the future. However, because I'm quite young (35) and don't have children yet, some doctors think I should just take a-blockers for a few years and after I start a family then consider BNI or Rezum
@Abhishek modi No, I am avoiding it at all posible costs to be honest. Next month I have a cytoscopy scheduled so that Dr can truly evaluate if Rezum would work on me: he says that if the problem is purely in bladder neck, then no alternative such as Rezum or UroLift is good because those treat the prostate. My urinary flow comes and goes in regards to potency, so I'm not in any pain day to day, but the cytoscpy results will dictate what I will do and how urgent it is. What about you?
@Abhishek modi scheduled for Rezum, which doctors say is worth trying. I'm on waiting list to do it through public health care system (Europe). What about you?
Sir, I am 53 yrs old having normal prostate size but with bladder neck obstruction problem. Though I am on alpha blocker and I wake up only once in night but the whole day I feel anxiety & pressure like at urine parts. Pls suggest why it is so?
Sir, I am in similar situation. My prostrate is only 33 cc. I have been facing frequent urination, noctoria, weak urine flow etc for 10 years. Custoscopy shows bladder neck obstruction. Medicines give me retrograde ejaculation. My PVR is more than 100 ml. I did PAE as well with no improvement. What is next course of action for me
Hey, I recently did REZUM (1 month ago) for the Exact same symptoms you have. Thankfully urine flow is much stronger, noctoria has gone down to just 1 time a night, did not affect ejaculation...but I still have PVR because I have to "push" when I urinate. Doctors have said that with Rezum I have to wait 2-3 months for swelling of prostate to completely settle before deciding if surgery was truly effective or not, but the cystoscopys have always shown my issue is the bladder neck itself, not the prostate. So we'll have to see if BNI will eventually be be be necessary. SUMMARY: Rezum was quick recovery and relatively painless, ejaculation unaffected, showed some improvements, but may not be the Ultimate solution to cure all issues
I have undergone for Bladder neck surgery after the surgery iam unable to hold more urine and urine is leakage little amount on daily basis? so what are the treatments are available if it's gone like incontinence?? Waiting for your answer please respond urgently. it's a big question for urology department what you will do if it's Accured incontinence after the surgery.
Have you seen doctor again? My doctor also says that if problem is only in bladder neck, than there really is no option other than BNI incision. Any updates from you?
@@shuckyducky5212 i have the PBNO and alpha blockers give complete nose block for me (to the extend that i'll have to breath only with my mouth).. hence, not taking alpha blockers.. now that i have recently undergone health check and the ultrasound is showing little different result than my earlier one.. my prevoid is 550cc and postvoid urine is 400cc .. which is too much of urine held back in bladder.. i got for another round within 2 mins and clear some more urine.. i'm now afraid to go with BNI as @siddiqbasha is having issue post BNI issue now (unable to hold urine)

@@shuckyducky5212 I think Rezum would not be a choice for me.. my prostate size is fine.. i think the issue is with bladder neck muscle.. and i'll probably have to go with BNI.. my recent ultrasonography says "Calyceal thickening both sides with Calyectasis" & "Bladder outlet obstruction with cystitis"
@@bharathdathi my prostate size was also fine (only 30cc), but surgeon said it was worth trying for now to alleviate some symptoms before deciding if BNI was absolutely necessary. For me, I think BNI will have to be final solution but i was worried about retrograde ejaculation so I wanted something that would give me some more time
