Demonstrating calf vein thrombosis has always been the biggest challenge for me, especially if the patient has leg edema or is somewhat obese. Seeing this video and finding out that other people search for the calf veins in sitting patient really give me hope of performing a better job. Good job.

I've been scheduled for a venous insufficiency ultrasound of my legs, I have blockages in 4 of the 6 arteries in both legs, also have issues with the veins bringing blood back up my legs... scares me on what might happen to me, esp when I have a almost 19 hour flight coming up sep of next year, that's just 1 way, and almost a 19 hour flight coming back home....anxiety is so bad, that if they need to roto rooter my legs, they are going to have to put me out as I can't keep my feet still long enough for them to do anything, I have a feeling I'm up poop creek without a paddle

I have high suspicion of calf blood clot but did twice ultrasound they always do thigh and foot but never scan the calf so for sure it seems OK for them.....

A good vascular sonographer can elicit your venous insufficiency with you in a supine position. Standing is not necessary. Hopefully you took a deep breath, held it, and tightened your belly like you were pushing out gas. This will unmask common femoral vein reflux and sapheno-femoral junction reflux which is where almost all insurance companies require the patient have valve reflux in order to pay for the procedure to fix your leg. For the rest of the superficial veins, the technologist can augment the skin with their hands, pushing blood up and down the leg to look at reflux times. This part takes skill and time that most young tech's in busy departments don't adequately perform. Most hospitals don't even look for venous valve insufficiency, most just rule out deep and superficial vein thrombus. Venous closure procedures are office based now and strippings don't occur as often and hospitals are disenfranchised to look.

My ultrasound tech grabbed (almost like a pinch but with her whole hand that encompassed a massive amount of leg) under my knee from the outside (it was super painful) and pulled toward the inside. She said it was to record flow. She did the same under my knee. If there was a DVT, would this not release it?
Yes, but that was needed to compress the vein or to push flow through the vein to show patency. If there was a clot then the technician would stop compressions after proving the exisistence of clot.
hi! ive been told that there is no need to angle correct the cursor parallel when it comes to VENOUS doppler exams, but i still from time to time see techs angle correcting -as demonstrated by you. would you mind clarifying this for me?
This is true because when we check the veins we generally aren't concerned with velocities, so no need to do angles. With arteries we need that 60 degrees or less to make sure our velocities are accurate.
Another point I forgot to mention. I routinely find significant reflux in perforating veins and treat them by different means, resulting in the successful treatment of varicose veins and their symptoms. Many of them are in locations that are impossible to be evaluated unless the patient is standing during reflux mapping. They can be at any aspect of the leg starting from the gluteal, para-perineal, posterior thigh, and obvious other areas. Now, please tell me in RT, when the patient's body is against the table, how do check these areas in detail with ultrasound? Impossible!. Only when the patient is standing on a high stool and the sonographer is sitting lower (on a low stool) all areas can be evaluated. the patient rotates and any area that needs to be evaluated will be in front of the sonographer. I bit that in practices that do the ultrasound with the patient resting in any position on the exam table and even in RT, areas such as gluteal, para-perineal, and posterior thigh are not studied well! How many incompetent perforating veins in gluteal, para-perineal and infra-gluteal and posterior thigh have you treated in the last 6 months. I bit almost zero! Changing habits is hard, I know it. But knowingly doing a partial ultrasound study in place of detailed and complete ultrasound is another thing. Most surgeons just focus on GSV & SSV, while there are many more perforators that can be a source of the patient's problems. Due to routinely checking all areas, in 2001, for the first time, I introduced para-perineal varicosities in multiparous ladies as a source of symptomatic legs varicosities that take certain routes into legs' superficial veins. And I introduced foam sclerotherapy of these varicosities which has proved to be very safe and effective. But the key is that one must think of these possibilities and LOOK for them, then treat! We don't know what we are missing, until flex our minds and start looking at things slightly differently. Then new horizons will show up. I have so many photos and ultrasound images of such cases that will make it clear to you what I mean. May be I should start making UA-cam videos about these points. Pictures say a thousand words.
Hello. Is it possible to have clots or dvt with normal d dimer? I have a tiny bruise on my calv but my leg hurts badly and I have weird sensations and pain in my leg.
This study is unfortunately not done with the patient in the standing position. All venous ultrasound studies for reflux evaluation MUST be done with the patient in a standing position. No exception. The guidelines are well published. Please do not undergo an ultrasound of legs venous reflux study if you are not standing.
Incorrect-If a patient has reflex that needs to be treated, it will show in reverse Trendelenburg with a good enough tech. If reflex doesn’t show in reverse Trendelenburg, than it is not significant enough to be treated. Standing every venous pt is tedious and unnecessary.
@@pblzr09 JK, you are wrong! Number 1: most practices do not even have a true reverse Trendelenburg table. Those tables take up a big space and are only used at particular radiology departments and are very expensive. Regular power tables cannot place the patient in the reverse-Trendelenburg position which requires a supporting plate on which feet rest. Using a regular table by raising the back and lowering the legs IS NOT putting the patient in reverse-trendelenburg position!! 2- The guidelines on Duplex Ultrasound in studying varicose veins, by the American College of Phlebology (new name: American Board of Venous and lymphatic medicine) specifically mention that "reverse Trendelenburg is not the proper position for mapping reflux sources in cases of varicose veins. Unfortunately, most physicians and sonographers out of being lazy and to avoid inconvenient themselves avoid placing the patient in a standing position. Guidelines are clear and in black and white. Sorry to be straightforward. Are you making it up that reverse-Trendelenburg is a proper position for studying reflux sources in cases of varicose veins, or do you have a solid reference? Please share your reference. We either want to do things right or bring excuses.
