Oh wow, a (potential) fellow Plague Carrier enjoyer! When Facets first came out, I played a lot of Patient Zero Veno, but it gets boring. I understand that PZ is just "free MMR", but I'd really love to get PC to work. I've been trying out this ability build: Q-W-E-E-E-R-E, basically maxing out Wards asap. The itemization can be whatever (I personally gravitate towards the basic bitch build of Greaves-Force Staff-Vessel). Pros: push power, safe farming (both for you in the jungle and for your cores, since you get a lot of vision), good objective control. Cons: it's not braindead Patient Zero.

During laning phase, move your camera and check the other lanes and heroes more, especially when you aren't busy last hitting.
