- This woman had vertigo and spontaneous horizontal unidirectional nystagmus to the left. Her HIT is normal when turned rapidly to the left. It's has a fairly obvious refixation (or catch-up) saccade with her head turned rapidly to the right.
She screened negative for central features, and had no vertical skew or hearing loss. She had R vestibular neuritis.
Nice video. Also, I really appreciate your clarifications on misunderstandings on the examination and diagnosing of vertigo.

Edit June 2024. I meant to say it is in fact right vestibular neuritis, and not left.
Can you explain how?
When patient head is first suddenly turned left than its normal but when his head is turned towards right than saccade is seen which corrects the eye by making it move towards left. This mean that the vestibule which makes eye turn left is abnormal
And i have read that the hyperactive labrynth pushes the eye in opposite direction so the abnormality in this case might me either right labrynth hyperactivity or left hypoactivity.
Is there anything wrong in this?

But i am bit confused. Isn't the right site affected? I have watched your video in the link: there is corrective saccade when head is rapidly turned to the affected site.
When this woman's head is rapidly turned to the right corrective saccade arises-> so shouldn't it be right vestibular failure?
