An article typically has a title, yes. In the introduction, however, you are flexible. A rhetorical question is a very nice tool, but if you want to start your article in a different way, you definitely can.

It absolutely depends on you. From my experience, three to four works best in most cases, but you are flexible with it. :)
hi, i had my camb test yesterday and i forgot to put a title for my article is this a very big mistake?
I would say it is one of the smaller mistakes you can make when you write an article so don't worry too much about it.
