The word derives from Italian fustagno 'fustian' and -ella (diminutive), the fabric from which the earliest fustanella were made. Fustanella as a dress is evolved from Ancient Greek dresses, and the closest direct ancestor are dresses from the Byzantine era, worn by Corinthians especially. The following is from the Sgraffito pottery fragments from the 12th century showing Greek warriors wearing the fustanella, from Corinth, Greece. This by the way is the earliest ever depiction of fustanella in contemporary times.
