my theory is that every time blue spy uses his building disguises he becomes more and more machine (that's why part way through his voice turns into a speaker) and the dream/tape is showing that he is now more machine then man (which explains why he was robot spy for a split second in the tape) and when the red engi died and spy got the cut in his head it broke the remaining part of blue spy's mentality (which was easier to break now he was mostly machine) and some version of red engi now lives inside of spy's head (which explains why blue spy now sees him everywhere) and happens to be the only thing keeping him human ,that's why we see robot spy get replaced as red engi in the dream cause it blue spy having a choice to ether evolve or remain and he chose remain and now that's a burden he has to carry

The reason he sees the red engie at the end has to do with psychological aftereffects of this battle. He keeps seeing the red engie, even his friends sometimes become him, which is a scary thought
I'm pretty sure that at the ending when the Spy sees the Red Engineer is actually the Red Spy survived their fight, and decided to look disguise himself as the Red Engineer as a way to mess with the Blue Spy's head and that's why we see him light a cigarette at the end.

The end is like where blue spy is fully in stasis, as nobody can hear or understand him anymore, and he is stuck to live his days in a hospital bed. He can only end the agony by ending it all, which he keeps seeing the red engineer from PTSD.

I've got a good recommendation for you. There is a video called the odd engies. It isn't funny but it is hauntingly beautiful and surreal. It was a saxxy entry back in 2013 but never made it. I would suggest it if you enjoy horror and mystery.
