Digoxin toxicity can manifest in a variety of symptoms, and nausea and vomiting are often among the earliest and most common signs. These symptoms may appear before other more serious signs, such as bradycardia (slow heart rate), confusion, or visual disturbances. A; Bradycardia: While bradycardia is a significant sign of digoxin toxicity, it is more likely to occur as the toxicity progresses and is not typically the earliest sign. C; Confusion: Neurological symptoms like confusion can occur with digoxin toxicity, but they usually present after more acute symptoms like nausea and vomiting. D; Visual Disturbances: Symptoms such as blurred vision or yellow-green halos around objects can occur with digoxin toxicity but are generally seen later in the course of the condition. In summary, nausea and vomiting are often the earliest signs of digoxin toxicity, making them crucial indicators for early detection and management.
maam this is related to question 13, does administering bronchodilator to asthma patient with SOB is an independent role of a nurse or still need a doctors order?
Administering a bronchodilator to an asthmatic patient with SOB generally required a doctor order as a regular medication or PRN medication. All medications that need to be administer to a patient in a healthcare needs to be order by the doctor and documented once the patient received the medications.
I failed my NCLEX exam 2 times first was 2022 second was 2023 and lost money on review questions and also wasted a lot of time studying which never helped me.

Hello, I advise you to contact Dr Luther if you are preparing for your nclex, be it RN or PN. He is the reason I and my husband are very happy today working as Rns🎉
Answer: B Emphysema is the permanent destruction of alveolar walls leading to airspace enlargement, loss of elastic recoil, decrease in surface area for gas exchange, lung hyperexpansion, and increased work of breathing.
CORRECT ANSWE IS A In clients with COPD, excessive oxygen administration can blunt their hypoxic drive to breathe. This can lead to hypoventilation, where the patient’s breathing rate and depth are insufficient to adequately exchange gases in the lungs. Hypoventilation subsequently causes an increase in carbon dioxide levels (hypercapnia), making hypoventilation the immediate concern to monitor for when administering oxygen therapy to these patients. While hypercapnia is a result of hypoventilation, the initial complication to monitor for is hypoventilation itself.
CORRECT ANSWER D ; Epinephrine is the first-line treatment for anaphylaxis as it can quickly reverse the severe symptoms of the reaction. Other interventions like providing high-flow oxygen, administering antihistamines, and starting an IV line are important and should follow, but epinephrine administration is the most critical initial step.
