Found Cummins to be absolute junk most require full inframe rebuilds between 240 to 300,000 miles. Your doing well to get more out of them. We have switched to Volvo and getting around 750,000 miles before any heads need to come off

How do you set the cutting tool
is there a step where you put the liner and shim in and measure the actual protrusion?
Yes you can set the liner in without the O rings and clamp it down with a bolt and washer to measure.
Where from I can buy height measurements tools
you can get those from monaco tool as well. Most precision tools for all things diesel can be found with Monaco or apex tool

Why not only cut it 10k and use a 10k shim ?
because 20k is the smallest shim that cummins makes. so it has to be 20k minimum.
