Answer: D. Down syndrome. The fetus has the classic "double bubble" sign of duodenal atresia. The increase AFI points to polyhydramnios, which occurs due the the inability of the fetus to swallow the AF. Duodenal atresia is strongly associated with Down syndrome. GDM and Rh isoimmunization can both lead to hydramnios, but are not related to duodenal atresia.

maternal hyperglycemia = fetal hyperglycemia leading to osmotic diuresis that will promote fetal urination (POLYHYDRAMNIOS)

A 25 year-old G2P1 female at 23 weeks' gestation was sent for US exam after her fundal height was determined to be 28 cm at the clinic. The US reveals an AFI of 32 cm, and the fetal abdomen reveals a cystic mass in the right and left abdominal region. Which of the following conditions is associated with these findings?
    A. Rh isoimmunization
    B. Turner's syndrome
    C. Gestational DM
    D. Down Syndrome

Maternal DM causes increased blood sugar in the mother which in turn results in increased blood sugar in the fetus which causes osmotic dieresis, therefore causing hydramnnios.

good presentation but the presenter's loud and constant gulping down his saliva was a bit...distracting. perhaps a glass of water before doing the presentation would have helped :-)
