The frequency of multiple pregnancies is increasing day by day. Sonologist must know that the correct, accurate, and timely determination of gestational age, chorionicity, and amnionicity as it has significant importance in the management of multiple pregnancies.
Dichorionic has totally different Surveillance, complications, outcome, morbidity, and mortality as compared to monochorionic pregnancy.
A major role in the increase of multiple pregnancies is due to the progress of reproductive technologies and in vitro fertilization. In fact, twins comprise about 3% of all live births in the United States. The majority of the cases of multiple pregnancies that occurred in the past were diagnosed during the intrapartum period. Today, as the use of ultrasound has become a routine in daily medical practice, multiple pregnancies are diagnosed much earlier by ultrasound scan. It is well-established that an ultrasound scan is more than necessary to define chorionicity, amnionicity, and gestational age.
A twin pregnancy can be either dizygotic (two-third of twin pregnancies), in which two different eggs are fertilized by two different sperms, and in this case, the pregnancy is always dichorionic-diamniotic or monozygotic. A monozygotic pregnancy occurs when an egg is fertilized by one sperm, producing one embryo, which can split any time, more commonly between day 2 and day 13 after fertilization.
The number of the gestational sacs and the number of fetal heartbeats in early multiple pregnancy scans is strongly related to chorionicity: each gestational sac will form a distinct placenta and chorion. Therefore, visualization of a single gestational sac with two visible heartbeats indicates a monochorionic twin pregnancy, while the presentation of two distinctive gestational sacs implies a dichorionic pregnancy. The number of gestational sacs is the parameter with the highest accuracy to define chorionicity which is extremely close to 100%.
The earliest your doctor would likely order an ultrasound confirming twins would be 4 weeks pregnant. Women who are going through fertility treatments or have early complications may be able to have an early twin ultrasound at 4 or 5 weeks pregnant and find out they are pregnant with twins.
With a twin pregnancy, the body makes high levels of pregnancy hormones. So morning sickness may come on earlier and stronger than if you were carrying a single baby. You may also have earlier and more intense symptoms from pregnancy, like swelling, heartburn, leg cramps, bladder discomfort, and sleep problems.
It is possible for twins to have different fathers in a phenomenon called heteropaternal superfecundation, which occurs when two of a woman's eggs are fertilized by sperm from two different men.
Twins detected at 6 weeks
It's possible to see twins (or more) on an ultrasound at around six weeks, though one baby may be missed at this early stage. Sometimes a heartbeat is seen in one sac, but not in the other. Rescanning in a week or two may reveal a second heartbeat, or the scan may show that one sac is growing and the other still empty.

how can you calculate your weeks and plan a scan according to it? sometimes women count 6 weeks based on trackers, or their last LMP, but when they go for scan, doctor tells them that your r actually 4 or 5 weeks. What do u suggest

Twin Pregnancy Ultrasound require expertise. Especially how to differentiate between Monochorionic and Dichorionic Twins on Ultrasound , that is very important to know as our further management in the antenatal period depends upon proper classification and differentiation between Mono Chorionic and Dichorionic Twin Pregnancies.

My sonographer couldn’t differentiate between an additional gestational sac and possible subchorionic bleed. They said it was two early to tell. Can a bleed be confused with additional gestational sac?

Hi....I had my periods on 2nd June. I did my frozen embryo transfer on 19th June. The embryo was kept freezed for about 6 months. I took my beta HCG on 2nd July. HCG value was 325.9. I did my ultrasound scan on 16th July. But ..only gestational sac of size 9.5mm and a yolk sac was found. There was no embryo and no heartbeat. Doctor told to repeat the scan on 30th June. Is there may possibility for the embryo to be visible and will I be able to hear heartbeat?
