Thank you so much po, medyo lumilinaw na po sakin pano gumawa ng ncp, baka po pwede mag request kung pano po mas mabilis magamit si nanda. Medyo nahihilo pa po ako sa difference ni Risk for and Deficient, parang lahat sila magkakapareho, parang lahat applicable 🥺

Hi po, we were given a case scenario. Pano po gagawin namin sa objective data? Ung given lang po ba sa case scenario which is the temperature only or mag assume pa po kami ng vital signs?

Nurse, meron pong “Risk for” sa NANDA. Hanapin nyo lang po then use the same technique kung paano ko po ginawa yung Nursing Diagnoses.

Nurse @@restythenurse totoo po ba na walang related to? as evidenced by lang?

    • @restythenurse

@@markvanocayatautho269 Meron po syang “Related Factors”. Pwede ka po gumamit.
Usually ang “Related to” is the cause.
      For example:
Yung pasyente mo is nagtatae. Pag check mo Wala pa naman syang dehydration, at ok naman yung mga electrolytes nya. Pero nag tatae sya, so pwede sya ma RISK for deficient fluid volume and Risk for Electrolyte Imbalance.
      Example:
Risk for Deficient fluid volume related to active fluid volume loss as evidence by loose watery stool.
Risk for Electrolyte imbalance related to deficient fluid volume AEB active fluid volume loss (LBM)
Like if mababa ang water sa katawan. Kasama din yan ang mga electrolytes.
You can actually have a “related to” when it comes to “ RISK FOR” diagnosis
