Because A is the area past the point you are interested in. So if you want the stress at the very top, there is no more area above the point you are interested in. So the A is zero making stress zero.
That is the dimension in the x direction at that location on the cross section. The “thickness” is the dimension perpendicular to V at that location on the cross section.
