Dr. Mudgil, I really like how you knead the tissue area to get the blockage to breakup and your small incisions to reduce scarring. I’m always interested in the history of the cyst, how long the patient has had it and such.

How much abcess launched onto thay big ole watch?
Medically, is there a benefit to keeping the incision made to drain an infected cyst or abscess so minimal? So many dermatological videos of the same type of infection always seem to be opened as much as possible to drain infection. Is it easier to pop the loculations with this technique? Or is it just as simple as knowing the would will need to be packed instead of sutured closed?
Last question: this is infection being drained, bacteria- right? Is it considered pus or just cyst contents liquified by inflammation? I hope this isn’t morbid, but when I see these types of videos I always wonder how bad the drainage smells but very new doctors seem to mention it. Is it for the patients benefit to avoid embarrassment or do they really just not smell the majority of the time?
All good questions! Making the smallest incision possible is always the best and most humane strategy. A repair isn’t required, which allows for additional drainage over the following days and any scarring is minimal. There’s really no reason to make a large incision. In this case, the contents were pus. Sometimes inflamed cyst contents can look similar, but there’s always history of a preceding cyst. Most don’t smell, but when they do, cysts tend to be more malodorous than abscesses - they’re typically hanging around for months or years, whereas abscesses are acute in nature.

Great job numbing it up, Dr. Mudgil! Abscesses can be quite resistant to anesthetics, speaking from unfortunate personal experience. Your sidestep when it squirted was great, a very quick reaction, but I'm even more impressed by watching you continue and finish the job before cleaning up. Thank you! This was actually a staph abscess, but cysts can get contaminated with bacteria, particularly if they are tampered with.
Great job but I do have a question. Why didnt you make the incision a little bit bigger cause it looked like it was struggling to come out while you were pushing down and shaking. Wouldn’t making the exit wound a little bigger fix that problem?
Thank you! Bigger incision, bigger scar. I’d rather put the work in and leave as little evidence behind as possible. My patient did wonderfully. 😉

Thank goodness I’ve always had these come up in places I can reach…I have no insurance and it’s way too expensive to have this done at the drs.

It certainly could be, especially if it’s tender and inflamed appearing. You should get in to see someone ASAP.
IMHO the incision was to small for that size cyst. Hence why once it was pushed under pressure by his squeezing it, it exploded..

I had a large cyst on my back and it would flair up every year. I finally had it x-scission procedure to remove it. Good riddance to that nasty bump.

The incision needs to be larger in order to remove the sack. The sack needs to be removed in order to keep the abscess from returning.
