My understanding is that an NC-17 rating in the US is seen as a deathblow and most cinemas won't show it at all. 18 is not seen the same way in the UK, even though it's the same in the sense that they both mean that you have to be 18 to buy or rent it or see it in the cinema. There is also the R18 category, which many people aren't aware of in the UK, because it's only used for hard core ₱0rn and films rated that way can only be bought at licensed shops or shown at licensed cinemas, and they can't be sold online
