The "bald Toad" is based on the old SMB3 cartoons, where Toad could take the mushroom cap off and had that little bit of hair on. But in the current game canon that isn't a hat. That's their heads. Koopas can exit their shells, and Yoshis are born with (and can remove) their shoes, so I suppose that doesn't mean Toad can't take his disgusting head growth off, but he's never been shown doing that in official Japanese media.
