The temporomandibular joint -- the TMJ - is the joint between the lower jawbone - the mandible - and the temporal bone of the skull. The TMJ is responsible for jaw movement and is the most used joint in the body. The TMJ is essentially the articulation between the condyle of the mandible and the mandibular fossa - a socket in the temporal bone. The unique feature of the TMJ is the articular disc - a flexible and elastic cartilage that serves as a cushion between the two bone surfaces. The disc lacks nerve endings and blood vessels in its center and therefore is insensitive to pain. Anteriorly it attaches to lateral pterygoid muscle - a muscle of chewing. Posteriorly it continues as retrodiscal tissue fully supplied with blood vessels and nerves.
The mandible is the only bone that moves when the mouth opens. The first 20 mm opening involves only a rotational movement of the condyle within the socket. For the mouth to open wider, the condyle and the disc have to move out of the socket, forward and down the articular eminence, a convex bone surface located anteriorly to the socket . This movement is called translation.
The most common disorder of the TMJ is disc displacement, and in most of the cases, the disc is dislocated anteriorly. As the disc moves forward, the retrodiscal tissue is pulled in between the two bones. This can be very painful as this tissue is fully vascular and innervated, unlike the disc. The movements made by chewing or even talking cause a chronic bruise to the tissue resulting in inflammation and pain.
The forward dislocated disc is an obstacle for the condyle movement when the mouth is opening. In order to fully open the jaw, the condyle has to jump over the back end of the disc and onto its center. This produces a clicking or popping sound. Upon closing, the condyle slides back out of the disc hence another "click" or "pop". This condition is called disc displacement with reduction .
In later stage of disc dislocation, the condyle stays behind the disc all the time, unable to get back onto the disc, the clicking sound disappeared but mouth opening is limited. This is usually the most symptomatic stage - the jaw is said to be "locked" as it is unable to open wide. At this stage the condition is called disc displacement without reduction
Fortunately, in majority of the cases, the condition resolves by itself after some time. This is thanks to a process called natural adaptation of the retrodiscal tissue, which after a while becomes scar tissue and can functionally replace the disc. In fact, it becomes so similar to the disc that it is called a pseudodisc.

this video is misleading stating that the condition will fix itself. while it is true that the clicking/popping may stop and pain may temporarily be relieved, what is actually happening is that the "psuedodisc" begins to break down and may eventually tear, leading to the two bones rubbing against each other creating a condition called crepitus and ultimately arthritis of the joint. All the while, the bone of the condylar head is remodeling and may make treatment extremely difficult. Please, seek treatment from a QUALIFIED TMJ specialist. Your jaw lock is a warning sign you do not want to ignore.
If you are really interested in learning more about how the TMJ works and treating TMJ pain I urge you to read the book "Break Away" be Dr. Fred Abeles

@@morganjherman I had a tmj dysfunction since I was 16, when I was 20 or smth it completely locked, I could barely open it at all. One day I got so pissed that I forcefully opened with all the strength I had and it fucking snapped. It was the most painful experience in my life. It then locked "open", I couldn't close it for like solid 20 seconds and then it all went away. It still makes a noise, but no pain... I wonder wtf happened.

@@Katerina-kqkq it's really weird. I don't think it snapped back in place, cause I still hear it crack whenever I open it "too much". But I definitely think it moved.
And it doesn't hurt at all. It's like before I would have 50% of range of motion and then it would crack AND hurt. Now it's like 90% is restored and then it makes a noise, but doesn't hurt. I guess I somehow made it move JUST ENOUGH to not irritate the nerve endings and move somewhat freely.

I was diagnosed with disc displacement with reduction 9 months ago from an MRI. Mine was caused by a prolonged dental procedure. it was an avoidable incident which makes me even more angry. The pain is a nightmare, but I've learned in time what makes it worse as to avoid extreme pain... eating certain foods, singing, laughing, excessive talking, crying, etc... Headaches, and jaw pain are the most problematic symptoms. I can tell you I've tried physical therapy, which helped somewhat, but the symptoms are still there. Now I'm seeing a specialist that made a mouth guard for me. That has helped especially when the headaches come on. I still deal with pain though. I can count on one hand the amount of days I have been almost pain-free since this has happened over a year ago. It's hard for people who haven't experienced this to understand the adjustments that have to be made because of this. Going out to eat, talking on the phone, visiting with friends and family are all dreaded because of the pain that comes with those experiences. Things a lot of people take for granted myself included up until the injury. This video gave me hope until I started reading the comments and saw that a lot of people still suffer after years, sometimes decades. Has anyone had improvement in time? I would love to hear a success story. My will to live, and well being has drastically tanked over the past year. I've been to 2 different dentists, 2 oral surgeons, a physical therapist, massage therapist, and a specialist since this happened. My hope is that one day there will be a solution. I'm sorry for all who suffer from this as well. I cannot believe how many people deal with this...some for most of their lives. I cannot imagine.

This is EXACTLY what i am going through! My jaw was dislocated bilaterally at the dentist and it's nearly 3 years later and i'm in so much pain. I have an occclusal splint that i pretty much wear all day but it's embarrassing as i cant talk with it in. I dread having to take it out, i dread going out with friends and family because of the pain. I've had so many massages its not funny. Physical therapy makes it worse. I feel like my left jaw just needs to be pulled over to the right for it to be back to how it was before the injury.

@@nikolafingarov9022 Oh no, I'm sorry as well that you are experiencing this. I can give a bit of hope. It's been 4 years now, and I have improved a bit. I've learned that certain activities will aggravate it, like singing, eating tough foods like twizzlers, yelling, and talking for long periods of time, so I try to avoid those as much as I can. It just is so crazy, we take advantage of the fact that we can talk, or yell, or eat our whole lives until something like this happens. I did start taking a couple things for the pain that have also seemed to help. One is a prescription called Paxil. It's for anxiety, and it can also be used for chronic pain. The other thing I take is over the counter. It's called Kratom. You can buy it online, or at smoke shops. I miss my life before the injury, but there is a little hope. Again, I am terribly sorry you are experiencing this also. I wouldn't wish this on anyone.

It took me a 1.5 years to finally recover from my tmj pain, partly because I didn't have a good strategy for tackling it. I probably would've overcame it sooner, knowing what I know now. The conservative treatment works best, but it takes times. You have to be patient. Here are my tips:
    1. Find a dentist who is knowledgeable about tmj
    2. Get a X-ray to determine whether it's an issue with your jaw muscles or disc. It's very easy to get the two confused, since they're close together. For a very long, I thought I had an issue with my disc. It turns out that I had injured one of my jaw ligaments located in the mandible area. The symptoms are similar to tmj. Please do your research on the jaw muscles, tendons, and ligaments to better understand the issue. Once you're better educated on the root cause, you can finally start tackling the problem.
    3. As I said previously, take the initiative to do your research. There are plenty of medical articles on this issue. Unfortunately, there aren't too many dentist who are well trained on this issue. Most won't be able to give you all the answers.
    4. Vitamin supplements such as magnesium, omega 3 , and other natural supplements that support joints, muscles, and brain are your friend
    5. Anti-inflammatory medication and muscle relaxers are a back up. Your go to should be Vitamin supplements. There are plenty of articles on this.
    6. Change your diet. Stay away from snacks and food that are hard to chew. It will upset your jaw muscles. Then, it will make the healing process much more difficult.
    7. Stay positive. Things will get better. There were times in which I felt depressed and became antisocial. The pain can be so bad that it can impair decision making. The pain effected my sleep for a long time. Find ways to keep your mind distracted. For me, I watched a lot of shows and movies.
    8. Work with your dentist and physical therapist to make sure that your jaw is aligned. I had to get a jaw adjustment.
    9. Rest your jaw as much as you can.
    10. Tens unit helps relaxes your jaw muscle.
After following these steps, I gradually noticed the improvement, and I was back to my old self. You have to be patient and endure the pain. Believe me, I've been there. Just stay positive and you'll get through this. Good luck.

I had this condition 5 years ago, with typical symptoms - pain, clicking and wasnt able to open my mouth more than 30 mm. Doctors was useless, 6-9 months only soft food. Finally, condiotion slowly start to improve and after additional 9 months I was able to open 48 mm.Never made MRI and never try mouth gaurd, jaw correction or teeths correction. Just suffered. Now, after 5 years no pain at all, I am able to open my mouth 51 mm without pain and efforts. Even, clicklling disappered, or it is present very mild, but in both sides, bearly noticeable. I am able to eat everything without restrictions and pain: steaks and other foods, but may be should be more carefuul. No grinding .Do not recomend mouth-oppening exercises , or may be very light. Do not want even to remember this period, and decided to come here again just to instill courage in all people with TMJ. No face deformation now, no saggy cheeks. But when was eating anly soft foods my face was of course with some deformation - I think it was because of lack of face muscles use. I hope pseudo disc formed, but no MRI confirmation or doctors conformation ever made, and do not intend to do that. My advice:
    1) suffering of 1 year you should just to endure
    2) aditional 6-9 months you should be carefull
    3) after that, if have no pain, live as never had this problem.

This actually helped reduce the #of clicks and the sharpness of the clicks too, but I still get locked sometimes and eating chewy things is hard. Recently I went to a chiropractor who told me the reason I have TMJ is from poor neck posture and EXTREMELY tight trapezius muscles (which I believe- it’s all connected!) The annoying thing about TMJ is, all these specialists are right but there’s just no single cure for it, and no one wants to claim it under their field. If anyone has had any other success i would love to hear because this is probably going to be a lifelong struggle.

yeah i feel you. i may not be as old but i have it really fucking bad. my OMS told me mines from stress. it’s been nearly 2 years and i genuinely just cannot get rid of it. my ears hurt so bad it’s not even funny. i can’t even wear headphones or put in earbuds anymore bc of how bad it hurts. plus, at this point, i have chronic ear infections. antibiotics don’t do anythign, it’s a fucking mess.
the one thing that did help for a little while was not eating hard foods and such, so perhaps that can give you some sort of relief? at least for a little while 😭

Sleep without a pillow. Correct posture tongue/body as much as consciously possible (don’t force tongue up). I had this after I had braces and I haven’t experienced any issues in the 2 years since doing these things. I think gym/pull-ups may have helped as well. I looked up trapezius muscle based on your comment and I work those out more than I did when I was younger.

TMD can usually be temporary. TMJ can last for YEARS. 12 years for me so far. This video has helped me understand it better thank you. I still don't know why dentist say it'll go away.. It doesn't... And there is pretty much no fix for it. Heat helps a bit. Exercises/massages help a bit sometimes but usually make it worse. But honestly it's getting worse every year for me. I can't even concentrate on simple tasks from the headaches of it. Does anyone else here have it as bad? I haven't spoken to anyone else with it who feels so much pain as well.

Wow, that explains why my tmj pain came and went over the course of about a year. Just like the video, it started out as a clicking, then my jaw started locking up some mornings until I massaged my jaw muscles. I started using night guards to help with clenching/grinding. Eventually the clicking and locking went away, but the left side of my jaw was just always sore, like 24/7. Massage and ice would help temporarily, but never for more than an hour or two. Eventually the pain started subsiding and eventually disappeared. My jaw still clicks from time to time, but I'm guessing this video described what happened. Who knew?! The body is amazing.

Over 20 yrs (on and off, multiple specialists, a neurologist, multiple scans, etc. ) and I finally have a diagnosis that my TMJD is tied in with daytime/night-time clenching, which is the source of my headaches.

This drives me crazy and I lose my appetite as the result. Sometimes the snapping sound turns into a crunching, ripping sounds that feels like some kind of tearing or rupture, and that's when it starts hurting.
It goes away overnight, but occurs again randomly. I only experience this on the left jaw; the right jaw pops rarely ever. The popping in the left is almost constant and might be a nuisance, so I avoid eating around people.

    • @Alilamedicalmedia
      @Alilamedicalmedia  7 років тому

It is believed that it does increase your chance of developing arthritis later on, but it does not mean that it always leads to arthritis. I dont have the numbers. What you can do is to be gentle with it, arthritis is mostly caused by constant mechanical overload on the joint.

    • @svetlomirivanov5303
      @svetlomirivanov5303 7 років тому

Thanks for the answer ! Will you explain further the logic behind this belief ? If new disk (pseudo disk) is ready, that`s mean that no bone-to bone contact (as in the case of original disk),so what causes arthritis ? Do you mean strong mechanical load as bruxisim,or even hard steak, for example?
I am from Bulgaria, and doctors here doesn`t use MRI to diagnose if the condition is TMJ Anterior disk displacement without reduction. I will discribe my case, please tell me if it is possible to be TMJ Anterior disk displacement without reduction or something other?
First of all, now both condyles DOES NOT go further across /forward/ the articular eminence, but stop just below,as I can see from my X-rays.
I am 44-years old male, my heigt is 187 sm.
      Here are the story :
Before 6 months I start suddenly to have a very clear reciprocal clicking on the left TMJ.I visited the dental surgery clinic and the doctor in duty told me I get anterior disk displacement with reduction (after manual examination only)
Two months later clicking stoped, but my mouth oppening now is 2 fingers. If I try wider,it start hurting. No deviation of the jaw, the insicors meet well in the center. It`s look like as anterior disk displacement without reduction now,but it is strange that no deviation, even to the affected left side, and very hard end feel, pressure provide no additional opppening,at all. Moreover after clicking stopped,my mouth oppening was normal (3 fingers, but hurting very bad, thats` why I stopped wide oppening, just to heal...Now oppening is 2 fingers, and no more). Local oral surgeons think that it is jaw subluxation, but this contradict two monts of reciprocal clicking and initial diagnosis . My face muscles look like a bit swolen, even after 6 months,on the both sides of the face.
I could send you my X-rays images and agree to publish and comment here,if you want,or on your web site
      Best Reagrds,
      Svetlomir Ivanov

    • @Alilamedicalmedia
      @Alilamedicalmedia  7 років тому

I cant do anything about your diagnosis, sorry. The association between disc displacement and arthritis is an observed one, not a logic. Once the pseudodisc is formed, the damage would be minimum. The mechanical overload would be different for different people, you would feel it if you force it, i would minimize the load as much as i can. Good luck!

i grew up chewing the back of pens and pencils since primary school, and the situation got worse cause i ended up cutting my nails with my teeth all the way to the University. Then i noticed a new behaviour, everytime i am stressed i bite my nails or chew a bubble gum or press my jaws very tightly. And a clicking sound starts on the Jaw (one side only). It got worse and stressful. Being Diagnosed was a major relief. The video accurately describe my condition. There is a additional information. Overtime i noticed that one teeth is being dislocated slowly over the years and the clicking sound worsens with every teeth misalignment. Solution for me is lowering my stress by quickly accepting situations, putting limits in everything, learning to say No firmly. Intentionally controlling my jaws to not contact each other when i am experiencing the click sounds or pains. "I still live with the condition but i have learned to control my jaws= relief. Thanks for the VIDEO. Is the most accurate.

I’ve had this for about 26 years. I only have it on my left jaw. The first 5 years were tough psychologically. Mentioned it to doctors but none of them knew what I was talking about because it was considered a dental problem. In the beginning the pain was sporadic but eventually got better. Eventually I learned to live with it. Now it just pops so I can open my jaw fully when I feel like it. Weird thing is a few years back, when I last got super high off of brownies I could feel my left jaw/ face like if I injured it but with no real pain. Hard to describe. It’s like my mind has been blocking that sensation all along but I somehow unlocked it with weed. Well that’s my story.

Treatment for TMJ after consulting atleast 5 maxillofacial surgeons
    1. Eat Soft foods.
    2. Take painkillers and muscle relaxants.
    3. Try not to talk much and give your jaw rest as the masseter muscles are small muscles and need time to heal.
    4. Avoid sleeping towards the affected side.
    5. Take TENS therapy and accupuncture sessions.
    6. Place moist hot towel over the affected area. Don't try to use dry hot water bags.
    7. Avoid mental stress as that will aggravate the problem.
    8. Take hot turmeric milk with black pepper. Strongest anti-inflammatory concoction available naturally.
    9. Eat healthy diet and take proper sleep.
You need to be patient with this as it will take time to heal but all the steps need to be taken.

Great advice. I've problem with my tmj for a year now. I followed these same steps, and my tmj symptoms started going away, but it takes times. Also, I've taken specific vitamin supplements for joint and muscles to help my tmj and jaw heal faster. My joint doesn't bother me anymore. Now, I'm just working on healing my jaw muscles. Also, it's very easy to confuse jaw muscle pain with joint pain. I got my X-ray done, and I got a physical examination by physical therapist to confirm my disc in intact. How long did it take you to fully recover?

I'm 14 I've had this for about a year now so I got it when I was 13. At first it only started with the clicking and occasionally the jaw would lock but it would go back to normal after a few minutes. A few months ago it started locking more frequently and it could take a few hours until it stopped but it never lasted long. About two months ago my jaw locked completly and wasn't able to open more than 25mm. It's still the same :'(((. I'm seeing a specialist soon so hope it will get better. It's extremely annoying when yawning and it hurts a lot if I have chewed on something for too long :(

The specialist said there was nothing he could do about it sadly.
I think you should go see a dentist so that they can examine your problem. If you have the same problem as me, it's going to be easy to fix so don't worry too much :) Also, try not to eat chewy foods and don't try to strain your jaw so that it gets worse. Hope it gets better for you :)

I understand that the pseudo disc can replace the original and get rid of the pain. However, in regards to the jaw locking and restricted movement , how does the Pseudo disc help in this case?
From what I can see, the original disc still remains in it's displaced position and will continue to cause difficulties in opening the jaw and allowing proper movement.
    Is that correct?
I have disc displacement with reduction. My jaw is tight with restricted movement and I'm sure it's effecting my speech. Getting rid of the pain is great but without getting proper movement back it is still not a optimistic feeling.
    Please can someone explain/ Thanks.

Like iron in my blood, learning disabilities, etc. and if I told my parents, they would probably get into a lot of stress and I don’t want that to happen. But after 2-3 years I feeling ok now, my jaw doesn’t hurt that bad, plus I am not even scared about my jaw clicking 😆. So I am totally fine, but after 2-3 years it’s still clicking. If you do, it'll just get worn with time and that'll make it sooo much worse than it needs to be. I know you probably won't be able to fully open your mouth otherwise, but it'll heal itself if you stop intentionally cracking it. Please don't make the same mistake I did lol
