would you recommend an implant for a 40 year old where pills no longer work well, but the problem seems to be mainly performance anxiety that has existed for over a decade? ive been on therapy for years but this anxiety never goes away and if you fail for whatever reason it just reinforces it and makes it stronger, ive tried injections a couple of times, and works, but my fear is priapism tho my dose is low enough, is also inconvenient to use, also read that injection eventually may stop working and you may need an implant anyway

Have you done surgery under local? I've had outpatient surgery and they used Twilight anesthetic and had no pain and wide awake after a couple hours.
We currently only offer surgery under anesthesia. We do administer a local anesthetic that controls the pain very well for the first few days.

Hello Dr Clavell, i have some questions:
How much did the durability of the implants improve in the last 10 years? How much did the infection rate of Revision surgery improve in the last 10 years? Can a malleable implant last 40 years without erosion or will it erode definitely in this long period of time?
1. There are several things that can influence in durability.
2. Depends on the surgeon but infection rates are low, even for revision surgeries. However, there are many factors that can put a patient at risk. Not a straightforward answer
3. VERY UNLIKELY a malleable will last 40 years. Some research studies suggsst Erosion risk is higher with malleables than inflatables.

Which malleable implants are you offering?
I advise you to use the Coloplast Genesis brace is the best brace so far, for Tactra it is excellent but it is difficult to bend and does not bend as required, Regicon its flexibility is very high and therefore the percentage of hardness is below average, while Coloplast combines rigidity and flexibility, in my opinion Coloplast is the best flexible brace so far and lives with you the whole life without downtime.
What’s your experience managing infections? Blind from birth from genetic disease that causes ED from birth. Surgeon said infection was 1 percent, infected 3 weeks later, scrotal hematoma, only happens .2 percent of the time. Surgeon did not use a drain, an operative mistake I think. Implanted April 2022 and removed June 2022. Surgeon tore urethra removing pump, allowed infection to become too advanced, Cath for six weeks. No diabetes, somewhat overweight but scholarly articles suggest weight is not a factor. Recently visited another, better surgeon to have a cysto done to check urethra for scarring. None, thank God. But I want another opinion, specifically related to corporal fibrosis. Despite VED use, my tissue is becoming hard and fibrotic. Sometimes it feels somewhat normal, other times after VED exercises, it feels very hard. The penis is buried from being explanted and a failed salvage during trying to wash out the infection.
The corpora is much thinner in width, an inch or so. Very embarrassing and my biggest two questions is will I end up with a permanent cath if another infection were to occur? Can you reconstruct the corpora with grafts to make it wider? Can I be imaged with an MRI to see if a surgery is even viable? SPL of 4.1 inches, 4.5 inches after first implant. My pubic fat needs to be removed which is a separate surgery. I’ve already contacted your office. Thanks so much.
