    Aspiration isn't necessary in the deltoid, there are no major blood vessels in that location, so aspiration in that area just causes more discomfort for the patient

    As a nurse, yes we hardly use the z track method. I appreciate the real world nurse teaching here!

      So how do know where to inject? I see some nurses place 3 fingers below the acromion process and then inject. How do you do yours?

      It is not recommended to aspirate for IM injections now.

      I think it is specific to the deltoid. ACIP states not aspirate when giving vaccines into the deltoid.

      @@chocolatefoo The recent book The Needles Secret by Marc Girardot presents evidence to dispute the safety of not aspirating in the deltoid. Dr. John Campbell just did a very interesting interview with the author.

      I don’t think it’s a really great video. I was wondering the same thing. I also noticed she didn’t wipe the area properly she brought back the germs to the center. And for teaching she should’ve put the used needle in the sharps container right away. I liked the deltoid injection thou

    When you use a circular motion to clean and go from in to out and back again isn't that bringing "dirt" back into the innermost part of the circle? Thsts what I see happening.

However, the effectiveness of cleaning in a circular motion depends on various factors, including the type of surface, the cleaning method, and the cleaning agent being used.
      In some cases, circular motions can be effective for removing dirt and stains, especially on relatively smooth surfaces. The circular motion helps agitate and lift particles, making it easier to wipe or rinse them away.

    Been having these for 8 years yesterday the nurse was being advised (trained ) on how to administrator the b12 in all of the of the 8years of having these this is only the 2nd time I have felt really bad pain in the muscle first time it was given by a doctor subsequent ones have all been by nurses and I often thought they were not putting it in the muscle as I felt no sensation not even the prick of the needle ,the one done yesterday had me gasping as the pain shot right up my neck The supervisor told me to move my arm to loosen the muscle ? today my arm still feels as though It's been bruised inside and during the night had to massage it just feel she put the needle too high up into my shoulder and hope this will not interfere with effect of the vitamin or anything else.

    Your point 8 ASPIRATION- please mention this clearly...you are not talking about ths at all - its only mentioned as point 8 on the piece of paper..not very clear!

      It's not a hard thing to do, it just requires you to be confident about it and making sure the person you're applying it to is calm and doesn't make something stupid during the injection.
      Tell the person not to move even if somehow they manage to feel pain, it seems obvious but I think it's worth mentioning, one time my dad suddenly moved his arm away from my hands and ruining a an entire flu vaccine because he overreacted.

    What about aspiration? Always aspirate to be sure there is no inadvertent intravascular administration of substances. Recent Danish studies have added aspiration to protocol. Particularly in vaccine administration studies show that there is an crease in clothing side effects after IV administration of substance (mouse studies) Please reference Dr. John Campbell,, PhD in Nursing

      Agreed! Additionally, when cleaning the area she DID go circularly from the inside out, then washed over the inside again with the "now dirty" alcohol. Then, no aspiration and then no massage afterwards. Did things change since I graduated?

      It is not recommended to aspirate IM injections anymore, so nothing she did was appalling, dangerous or unacceptable. Shame on you.

      The Centers for Disease Control and Prevention (CDC) no longer recommends aspiration before intramuscular (IM) injections. Aspiration, which involves pulling back on the plunger, was previously standard practice but is no longer recommended for several reasons:
      The injection site doesn't contain large blood vessels that could be a risk of intravascular injection
      Aspiration can be painful
      There's no scientific evidence to support aspiration
      Auto-disable syringes, which usually don't allow aspiration, don't seem to increase adverse events
