Hello. Thank you for the inservice. I have a question about MEAT criteria. In the inservice, you state that some chronic conditions do not have to meet MEAT criteria. Can I assume that is referring specifically to the scenario you presented and not in everyday practice? I am a primary care practice CDI specialist and it is my understanding all HCC diagnoses have to meet MEAT criteria to be eligible for risk adjustment. Can you please clarify? Thank you.

Hi Ms.jay, I learned a lot from your videos. It helped me to clear my ccs exam. Thank you so much. I am a new hcc coder now. I am extremely happy to see risk adjustment video from you. Can we code the encounter which doesn't have vitals? Because in my current project we don't code the encounter which doesn't have vitals or PE .the reason is lack of evidence for face to face. Please clarify this.
OH Wow that is awesome to learn. Congratulations. We will have to contact our risk adjustment specialist for that answer. BUT I do know that yo should always follow the guidance of the training provided by your employer, because some follow different risk adjustment rules.
Do you have to pass the ccs to be a HCC coder? Or can you be an Hcc coder with the cpc credential (i know this video was a while ago but I am currently taking the cpc)

These pertain to provider documentation standards: S Subjective part of the note, O is the Objective part of the note, and A is the provider's assessment, as well as P is the patient plan. CHEDDAR also pertains to physician documentation. C is chief complaint, H is history, examination, D is details, drugs and dosages, A is assessment, and R is return visit.
