    I know exactly how you feel. I worked with a computer entering data 8+hours per day. I got progressive lenses and wore them for many years. They were great. Then I developed cataracts and very quickly they progressed. During the past five years I have been ill with calciphylaxis due to medication. Had ulcers and did wound care for over a year. Doing ok with this issue then I developed calcification in my kidneys and my Aortic Valve. Had an Aortic Valve replaced in February in 2018. My point is I was on medication for multiple years and thought my unstableness was due to all of this happening. I have finally realized that my cataracts have progressed so fast that it is now time to have the cataracts done. In the next few weeks ?I will have them both done. Thanks for your videos. I have learned more about this procedure and I am no longer apprehensive. Great job with your videos. I did watch the other one about the surgery itself. Thank You Doctor Shannon Wong. GREAT JOB 👍😁👏👏👏👏👏👏

    Hello Dr. Shannon,
    First I want to thank you for sharing your experience here! I am about the same age as yourself and have never had eyesight issues until recently (past 1-2 years). In fact until just the other day I had never even had (or needed) a comprehensive eye exam. I would say my current eye sight is very similar to your preoperative situation. When the time came I just went and bought readers, I started with 1.0 and over the past year I am now up to a 2.0 and can't really read anything small up close without them. However, like yourself my intermediate and far sight is perfectly fine. I am a medical imaging service engineer and struggle with the same frustrations as you had, only I am not always in one location and readers can be a real hindrance at times (especially when I forget them :).
    I went to an optometrist yesterday for an exam and to see what my options are, given my age he also recommended lens replacement. He discussed the Symfony lens option and also just doing a standard mono vision lens. In fact he installed a near vision contact lens so I could test it out and see if the mono vision option would work for my situation. My right eye is my dominant and he put the near focus contact lens in my left eye. Sure enough I can now see fairly well close up using my phone and computer without readers. The funny thing is he put contacts in both eyes but it seems that only the left is actually doing anything. He did not mention the possibility of just doing the one eye, and I did not think to ask. When I go back tomorrow I will certainly inquire about this option.
    He also said that with the Symfony option my near sight would improve dramatically as well as retaining good far sight, but that the intermediate sight would be an issue so I would still need glasses for intermediate vision for things like viewing a computer screen at distances of 2-3 feet away. He also mentioned the potential artifact issues with bright lights at night. So he felt that if the mono vision works for me with the contacts that would be the preferred option .
    My questions for you are:
    1. Since like yourself I only have near sight issues, why would we replace both lens's? Especially for the mono vision option?
    2. Did you experiment with contact lens's to determine the best option?
    3. Can I get a contact lens that would simulate the Symfony lens to see what that would be like before actually having them surgically installed?
    4. Since your far sight was also good why did you choose the Symfony lens over the single focus lens?
    Basically if you would please discuss the various options with the pros and cons of each, and why you chose the option you did. Do you believe you will have to eventually do the other eye as well or do you think it is possible that this will be a relatively permanent solution?
    5. They also mentioned that fine adjustments may need to be done with Lasik, did you need to do this also?
    I did find it odd that my Optometrist pulled out a pair of readers during the consultation. He looked to be well into his sixties, and I did ask him why he had not corrected his own eyesight yet. He did say that he plans to do it at some point before he retires. Watching your video contemplating your own surgery certainly helped me understand why some folks would choose not to do it. However like yourself I believe that the risk is very small and the potential benefits far out weigh those risks IMO, especially when it is for one eye. I have already decided that I want to do some sort of corrective procedure, just trying to figure out what would be the best option.

      Eric, Each individual will have unique and subtle differences that we identify during an exam. Then there is physician bias and opinion which will be unique and different between different ophthalmologists. I can only state that the video and the part 2 video of my experience 2 wks after surgery remain accurate now 19 months after surgery. Nothing is perfect. But presbyopia and readers suck. I would do it again in a heartbeat.

    Dr. Wong, after the implant of the Technis Symfony Toric IOL in your right eye, you now have very good near vision and can read very small print. You probably have J1 or J2 near vision in your right eye. Questions: What is your distant vision in your right eye with the Symfony Toric IOL? Do you still have 20/20 vision in your right eye? What exactly is the distant 20/xx vision in your right eye?

    I am a software developer who works w/ Symfony web software. I think I'm now sold on the Symfony eye firmware too. I'm assuming it's a sign from above. We'll see how this goes in 2 months....

    Dr. Wong, I'm curious about near vision quality and looking for some measurable result to expect... I have a Snellen chart that is meant to be held at 14" and lists J1-J10.... what should the average Symfony patient expect using this?

    Dr Wong, or anyone else who has had the Symfony lens implanted: I am considering the Symfony. I hear that there is an effect the contrast of what you are seeing. I didn't hear that addressed in this video. I am an artist who works on oil paintings. Reading clearly is important but so is seeing colors and contrast correctly. I heard that one patient reported things looked "waxy". Dr Wong, you said said after surgery that everything looks great. Would you say there is any noticeable difference in contrast? Or do you feel you seeing very well overall?

      I see no loss of contrast in my eye or in the eye of 99%+ of the patients that I implant this lens in. In my opinion, the symfony lens for most individuals works as well as a 40-42 year-old human lens.

      @@ShannonWongMD Thank you very much for you prompt response! I want to try the Symfony lens but I have heard some complaints. I suppose people can have problems with any lens. And maybe people who have a problem are more likely to speak out than people who are happy. Still I have heard several people say that they can't see clearly with the Symfony lens. I sure wish we could try these on like glasses. Instead we have to jump in and make a big commitment, unsure of the outcome. Thanks again for your feedback.

      Night vision is fine. There is an artifact that is seen around car lights and street lights -intraocularlensexperience.files.wordpress.com/2015/07/dt-symfony-left-eye.jpg - Overall "completely satisfied" with my vision. As my left eye sees perfectly far away without glasses and I am 99.9% glasses free currently, I will leave my left eye alone.

    Hi, Dr. Wong. I'm considering getting the Symphony Lenses for both eyes. How closely can you see with them? If you hold something up to your eyes, how many inches can you bring it before it goes out of focus? I was told by my optometrist that the symphony lenses don't focus as close as the old multi-focal lengths. So how close can you see? Thanks.

      I advise patients that with a symfony lens most individuals read clearly 18" away from their eyes. For vision closer than 18" you may need reading glasses. Hope this helps.

    Surgery yesterday for symfony. Once night came bad problem in house with glare, halos and ghosting watch tv and some lights inside house. Will it stay like this? Could it get better. Looking outside at streetlight see massive halos and glare

      Vision the day of surgery is always bad. Wait until the day after surgery for a better assessment.

      Shannon Wong, MD thanks for reply. And your videos. Wish I had found them earlier, I’ve shared them with my cataract surgery Facebook group. I think they will answer lot of questions people there have and best representation of what surgery is like I’ve seen in any of research I’ve done.
      I’m trying not to panic yet, but it’s hard. My vision in daylight hours has amazed me almost from right after surgery, it was just when it got dark problems happened. And still seems same today if go in darkened room. With both eyes open it’s not too bad, but when just look thru surgery eye it’s bad. Was going do other eye soon, but now afraid to also put this symfony in it.

      porter Same here. Indoors the glare is very bad, and I'm 3 months out from the Symfony in my right eye. My doctor said the brain will adapt, I just thought it would have adapted by now. I cannot imagine doing the left eye with a Symfony right now, so will just reassess the situation next year.

    I am a post kidney transplant patient... Over the years after transplant I developed cataract in the middle of both eyes due to intake of low steroids...
    It's affecting me a lot and now I opted for Multifocal lens (Symphony)cataract surgery...
    It's been 8 days now after surgery to my left eye..
    M seeing too much glare and halos around lights...
    Finding difficult to drive at night..
    I can see little better with my both eyes open...even though my right eye too need surgery...
    As you explained I am seeing shakey view through the left eye...
    And left 15-20° view of my left eye from left side is blurry..
    But my intermittent and little far vision is sharp and good
    So my question is....
    Which lens should I opt for my right eye... Confused bcoZ now from Multifocal lense I may not see distance vision clearly as halos and glares present...

      Hi sunil, I was reading your comment about the glare and halos while driving at night. I am curious as to how your vision has improved. I need the surgery on both of my eyes due to cataract and I am trying to find out which lenses should I get. I am nervous about the multifocal lense because of the halo and glare.

    An excellent discssion. I have 20/20, and no problem wearing a plus lens for all close work.
    It is always an intelligent trade off.

    Shannon Wong, MD, IT HAS BEEN 2 MONTHS OF MY RIGHT EYE IMPLANT ONLY AND FROM THE BEGINNING OF THE IMPLANT. THESE ARE THE PROBLEMS I HAVE EXPERIENCED WITH THE SYMFONY TORIC LENS. IT HAS ROBBED THE BEAUTY OF LIFE AWAY, NOTHING LOOKS RIGHT.
    1. Dirt is highly defined. Dust particles are pronounced. You would need a cleaning cart with sprays and rages to clean everywhere you go to make things look pleasing.
    2. Looking at rusty things is over defined. It looks like a mold build up and very unappealing.
    3. The gas flame on the stove is no longer light blue. It is dark purple blue.
    4. The paint on cars is iridescent. The paint glows like candy apple red, blue, green, etc. What you also see is a highly shiney gloss coat which over rides the color of the car.
    5. Can no longer see the beauty of my kitchen cabinets. you no longer see the finish of the wood but the black grain of the wood pronounced taking away the beauty of the finished wood.
    6. The yellow tiles in my bathroom are no longer yellow but more on the white side where the simulated grain also is pronounced taking away the beauty of the tile. What you should be seeing is the yellow/beige surface of the tile and the faint pattern of the simulated grain coming through.
    7. Driving at night you see a halo around the light with multiple rings like the rings when you count the age of a tree. Also through the rings from the center of the light are lines like when someone draws a sun and they put straight lines shooting out simulating rays. The lines spin as you turn your head.
    8. To read a paper you need to be distance it at a full arms length. Can't see it up close blurred.
    9. When I walk my dog in the church parking lot across the street from my house, I can no longer see the red mulch in the garden beds outside the church. I can see the colors with my left eye, but no longer with my right eye.
    10. The grass blades are highly defined where I can see through them to the soil and the color is not right.
    11. the soil is no longer brown like before like I see it with my left eye. It is black and the dead grass is not yellow brown the way it should be but white. The light brown hue is also cancelled out. The green grass and leaves are not the right green shade and you can see the over focused grass blades and you can see through them into the dirt.
    12. The original hue of my eye is yellow upon the background. The new lens is white - blue and nothing looks correct. Hue is one of the main properties (called color appearance parameters) of a color, defined technically (in the CIECAM02 model), as "the degree to which a stimulus can be described as similar to or different from stimuli that are described as red, green, blue, and yellow",[1] (which in certain theories of color vision are called unique hues). Hue can typically be represented quantitatively by a single number, often corresponding to an angular position around a central or neutral point or axis on a colorspace coordinate diagram (such as a chromaticity diagram) or color wheel, or by its dominant wavelength or that of its complementary color. The other color appearance parameters are colorfulness, saturation (also known as intensity or chroma)[2], lightness, and brightness.
    13. When you look at the concrete, the stones are very pronouned and you see every inperfection in the surface. technically this lens has taken the beauty out of life.
    14. People skin tones are blotchy and not correct. You see different shades. Black beards with grey, the grey jumps out making people look much older. Women who are skinny, you can see the cellulite on there legs and the knees area is ugly.
    15. I recently looked into a steam table and the food looked real bad unappetizing. When i make pizza at home which i have been making for 15 years every Sunday it no longer looks appetizing. The red sauce looks fake and the cheese and crust don't look right.
    16. When I look at a new stainless steel sink which is brushed. You can see more defined where the brushed lines are no parallel but going in different directions. i have been instaling these sinks for 35 years and never saw this detail which the manufacturer doesn't intend you to see.
    17. My bathroom sink, when I bought it 8 years ago was a light amber yellow color. My wife wanted to remove it, and replace it with a white one to match the tub and toilet. When I tried removing it, the silicon was so strong I left the sink as it came. Today when I look at it through the new lens it is completely white, the yellow is totally filtered out. With my left eye it is yellow. The yellow hue is gone and replaced by a white-blue hue.
    18. The night light in the house puts a reflexion on the eye lens. At times not always light reflects of the lens.
    19. My black tan German shepard doesn't look right his hair is to course locking and the colors are off.
    20. Women don't look pretty anymore.
    21. When I look in the mirror my mustache and eyebrow hairs are not defined. Problem seeing the strands of individual hair.

    22. You can see indepth imperfections on newly painted surfaces like drips, orange peel, lines, etc. I panted for years in my youth and never was able to see this detailed..
    23. Women spend a few $100 on hair coloring. When you look at the dye jobs they look very bad and I can pick up shades of other colors.
    24. If I eat potato chips. When I dusted off the crumbs. I can see tiny crumbs stuck inside the weave of the fabric which doesn't come out easily even after patting the clothes down a many times.
    25. Using the computor at times not always I can see a straight line going throught the sentances. Also at times slanted lines..
    26. Can't see the date and heart rate on my fitbit watch screen to good, only the time which are larger numbers.

    27. Faces on old TV shows look washed out no pigmant like rosey red or tan.
    28. Last Thursday I went back to a job i was doing for a contractor which is in the finishing stages and the new oak floors looked real bad. For 20 years I have been seeing his finished floors and they are like finely finished instruments. The contractor is Hungarian and takes big pride in his work. Once again the black grain is very pronouned looking like ants waiting in a straight line. Your don't see the floor finish first and the grain underneath faintly coming through as i have seen all my life. To put it quite Frankly they look like shit, horrible looking.The contractor is also a high end cabinet maker and the light wood blonde finish wall unit book shelves he installed you see the wood grain come through like mountain peaks. I've been seeing his work for 20 years and never saw anything like this.
    29. Night driving the glare has become worse now from the time the lens was newly installed from the on coming traffic making it more difficult to see..
    THIS NEW LENS IS A GIFT AND A CURSE AT THE SAME TIME.

    I would do nothing for Presbyopia.
    Can read computer ar 26 inches.
    I Personally confirm 20/20 on my Snellen.
    Surgery is high risk.

    Hi Dr. Wong...… You answered a previous lens replacement question for me on a different video. Thank you. I know you cant give specific medical advice without seeing a patient but any general thoughts you can share I'd appreciate...... Ok, here goes: I'm 50 with a cataract in my right eye. Left eye no cataract. I'm trying to choose what lens to go with for surgery. One ophthalmologist said he'd only put me in a mono focal because of my young age and left eye is still good ( apparently multifocals work best and are adjusted to best by patient when put in both eyes). A second ophthalmologist wanted to go with a multifocal...…. And I see in this video your colleague seems to say between the 4 to 5 minute mark, that he wouldn't think of putting you in a Symphony if you weren't an eye doctor already...… The Panoptix lens is out now to much acclaim... I'm moving forward soon and need to make a decision. Final bit of information is I've been to a retina specialist and all is good there too...….. So is there any general thoughts you can share on the topic of young patients with a cataract in only one eye? Thanks...….

    I am set to get at least one Symfony lens in two weeks. I only have a tiny cataract in the other eye and MIGHT wait. But, did you notice any issues with object size with only one IOL? My doctor thinks I should go ahead a do both eyes, because my brain won't like the object size difference. But I've heard of a lot of people with only one IOL and they never complain about this issue.

    Dr. I had the Symfony lens implants at age 69, 2.5 years ago. I was so surprised how good my vision was restored to! I love the Multi-focal lenses and now at age 72 I still do not need any glasses at any distance.

      POD 1 went back to work. No problems. Optics are basically better than my 49 you natural plano presbyopic untouched eye.

    So, does this thing CHANGE FOCUS, allowing you to follow something from 6 inches in front of your face out to 1000 feet away?

      Functional reading vision at about 18 inches from your eyes, excellent intermediate vision (about 30-40" from your eyes) and excellent far vision.

      Shannon Wong, MD So it’s three fixed distances, not a continual change of focus like a normal lens. I read about some Russian FLEXIBLE IOLs that provided a return to the vision of your childhood. That’s what I’m waiting for!

    Mr Wong if you read this I am 56yr old female. I had tecnis, Toric aspheric IOL at 55 years. Had YAG procedure about a month or two after that. I see halos concentric circles at night sometimes during the day very debilitating . I saw one of your procedures where you did lens exchange after Yag.i was told I could not do that. But you did, can I.

      Possibly....you would have to see an ophthalmologist that is experienced with intraocular lens exchange. Would require a long conversation involving risks/benefits that you would have to weigh.

      Yes. It is possible to remove a lens implant after yag capsulotomy and have it exchanged/replaced with another lens implant.

    Shannon, I learned of this video today.....and I was already scheduled for surgery next week. Your video made my decision to do clear lens exchange firm. I'm excited to get it done. I'm 51 and my colleagues say I'm crazy or to wait. With all of the testimony of so many patients doing so well, the risk is small just based on the last 12 months of the Symfony introduction. This quick adoption by the industry proves this lens is amazing. It will be 10 years before anything surpasses it. I'm not suffering for the next decade waiting for something better!     I'm a month out from having a Symphony multi focal implanted, in my left eye, and I Hate it so far! I lost being able to read without glasses, which I had great reading sight before. Just had a cataract, and had astigmatism, so I couldn't see far distances. NOW?...
    Among other issues I'm having, I still can't read street names very well, and need glasses to read now. I also have annoying starburst lights. Think before it through very well before doing it! At a 2.800.00 upgrade cost, was it worth it? That's a resounding NO!!

      I've heard this lens takes a while for the brain to get used to. Is your vision any better now, 3 months out?

      Your lucky my mine cost 4300. Ani too can’t read with out readers and still can’t see far away which why I had done.

    Dr. Wong, thank you for sharing your experience with only one Symfony Toric IOL. I am hoping in the future when my left eye's cataract gets worse enough to affect its vision to get a Symfony Toric in that eye to go with the Tecnis Toric monofocal I got in my right eye about four months ago. My right eye had a cataract that had gotten worse enough the past year to reduce best corrected distance vision to 20/50 or so, but it had too much astigmatism for the current max cylinder Symfony Toric, so I went with a Tecnis monofocal toric ZCT450 in that eye for good distance vision and I achieved the target -0.25D with

      I Have no idea if or when symfony toric models with cylinder correction>375 will be available. If you have more astigmatism, than what a ZXT375 will correct, the residual astigmatism can be corrected with astigmatic keratotomy or LASIK surgery using currently available technologies.

    I have symfony lens put in on April 10 when you tested my eye the next day I could not read any of the numbers.You told me my eye was very dry and I told you about my dry eye condition . It is April 23 2019. I still can not see any better close up or far away. Ivan only read with my readers.
    I live in Tucson and was going to wait a month to do my other eye but sometimes I see better with eye so I. Am very afraid to do the other eye.
    I did go to my eye dr a said eye was very dry and goo job putting lens. I know you said some times you have tweet. I know you can’t say anything with out seeing me but I need to know when and what I should be able to see. I am so disappointed. Others had said that their experience was great. I don’t want to go there if nothing can be done. Sereno. 08 26 51

    Fantastic video Shannon and congratulations on a great result! I saw that you had a little WTR cylinder on the ORA refraction -- did you end up treating that? Did you target slight myopia, and what is your residual refractive error?

      We were targeting -0.25 to -0.50 OD....have not refracted my new eye since POD1. On POD 1 my OD symfony had 0.25D cyl....not visually significant.

    Hi Out of interest how is your Symfony lens vision when using your microscope and looking closely in a mirror.?
    I am also 49 yrs old and have left +4.5D and right +3.5D and due to have Symfony ZXT150 27 power in my left first to assess the night artifacts before deciding on a 2nd Symfony or a monofocal set for distance.

      That`s great to hear,
      We are aiming for -0.89D so any possible further Lasik refinement to bring within -0.5D to -0.75D will be easier if in fact needed.
      I am having the surgery later today !

    Thanks for sharing your experience! I am an optometrist who just also just had Symphony on my OD 2 weeks ago. Quite an impressive lens. I would say the quality is just as good as my multifocal soft lenses. I'm due to have the OS done with Symfony in 1 week. I still have -1.00 residual WTR cyl. that doesn't really bother me when viewing binocularly Do your patients get even better reading with the 2nd eye done with Symfony? I'm a little hesitant to do Technis on my 2nd eye as I like the night vision with my Symfony. What do you think?

      I find the reading vision is J1+ with a refraction that is -0.25 sphere to -0.50 sphere. I am -0.50 sphere in my symfony eye and 20/20-1 J1+. My phakic eye is 20/15 UCVA (but J7-J10 near).

      I ended up doing Symphony on my left eye and couldn't be happier. Vision is J1 up close and 20/20 Distance OU.
