Demi had to get in her car to leave the alley so Miley could leave when her car arrived. If you listen closely Demi doesn't close her car door til Miley's car arrives for her. She waited as long as possible before leaving.
This is why I like Miley better than Selena because she actually talks to paparazzi and tells them to leave, while Selena just ignores them for attention.

@1222Kassey If you haven't noticed, she is one of the nicest celebrities to the paparazzi, of course at times they will get impossible to handle and someone will lash out like this, but look at any other video and Miley handles the paparazzi unbelievably well.
