Wait, did you miss one of the environment puzzles from up high at the castle? After redoing the blue pressure-plate puzzle, you didn't do that one from up above when you went up the tower the second time. Do you do that one somewhere else in the run? [Start at about 31:40 to see the section in question.]

Simply by observation... You look what could be a puzzle... And try to make it happen.

There are some puzzles that have a weird effect with one of the symbols that causes a failure at first and then a cancellation of that failure.
