All depend on the direction of the stab wound through the eternal angle. For example if it is straight from front to back you have a list of vital structures behind it: Aortic arch, Trachea birfurcation, Pulmonary trunk bifurcation, Left recurrent laryngeal (and Ligamentum arteriosum), Azygos vein, Nerves - cardiac plexus, Thoracic duct. However it can affect heart and pericardium if it is tangential downward
