I appreciate how you described that bipolar does not oftentimes look like what people expect it to look like. I have long maintained that I don't think I have ever had a normal period of time. If I'm not hypomanic or very depressed then I'm kinda depressed. I'm never "okay". Also, my levels of hypomania and depression are not the same with each phase and has varying lengths of time as well. Thank you for talking about that.

I love my friends,,,but I hate being around people...,,and with my job,,people think I want to hear and solve their problems,,,I dread going to work everyday,,,everyday,,,its like pulling teeth,,,Its to the point that I get sick,,,anxious,,,and even panic feeling,,,I don't like feeling the way I do,,,I use to like being around people,,and love my job,,,I guess after 39 years,,,its time to change jobs,,,because I am sick to death with the stressors,,,of people thinking I can,,or even want to hear their drama,,,I have enough of my own,,,sometimes I really want to tell some of them,,to shut the hell up,,and get the hell OUT...
Ronnie Evers Hypomanic? Just so you know lithium, Depakote, and the atypical antipsychotics like risperidone (I wouldn't recommend Zyprexa as it causes too much weight gain) are best anti-manic agents. Lamictal is more anti-depressive and can be slowly increased to about 150mg a day depending on how you respond to it. The slow titration (increase of dose) is important to avoid the potentially life threatening rash it can cause, but you've probably been told about that. Also so you know, once you go on lithium and stop studies show you're worse off than if you never took it, so if you do try lithium try to give it a good amount of time before deciding to stop it, if you think you should. I'm 26 and bipolar or schizoaffective depending who you ask. Hypomania is a state I enjoy but it can be irritable and unenjoyable for some people. Women tend more to feel suicidal when manic, and men tend to be aggressive. -Chris
Thanks Chris..but I know..from what I have reserched that lithium is not a options for me because of what you said too...I seem to be doing better..and Dr now has me taking 200mgs at night of the lomotrigine..and am comfortable with it..so far..and was told about the Steven Johnston rash possibility. Note if it were demonstrated that biopolar disorder were fairly common and it appears that bipolar II is, then that would justify more funding being directed towards its detection/diagnosis, treatment, and management.
I'm interested in the role of electrolytes in the pathogenesis of Bipolar: potassium, sodium, magnesium and calcium. Electrolytes are acquired through the diet.
I know a number of people who are afflicted by many of these symptoms of bi-polar disorder. I feel like I have to be cautious at times when communicating with them

I feel I have to contribute my opinion which is totally non-academic but it is a fundamental issue with mental illness. Therapy and medication can cost astronomical sums in many, many parts of the world and usually mental illnesses require long-term treatment. It is a vicious circle - financial limitations hinder treatment and treatment is expensive and causes stress and exacerbates the disorder and is often left untreated because of the high costs it incurs.
This video is 9 years old but I was surprised to hear about Lamictal. Just started Lamictal about 2 1/2 weeks at first 1/2 of 25 mg now at 25. I understand about the caution of increasing Lamictal too fast. I thought it was a very good video and definitely explained about different medications and how they can react with each other both good and bad. Having said that you didn’t mention how long does a medication take to see results of improvement in the patient. The other issue Dr. Appleblaum is how can you diagnose a patient in 15-20 min interview/ assessment, I would think you are being disingenuous to your future patients by making such a quick diagnosis. All in all I have learned much with your presentation and I hope in the future there will be more on the subject of Bipolar condition.
Dr Applebaum was incorrect when he suggested when assessment reaches Bipolar vs ADHD the next step is it a mood disorder vs. cognitive disorder. ADHD is much more than inability to concentrate. The most problematic clinical feature is inability to modulate emotions, and more simply impairment of impulse control. Bipolar patients will give histories of severe or Major Depression while ADHD very often causes depression but not commonly Major Depression. Many patients suffering from ADHD will not have any significant history of depression. Another difference is Bipolar is episodic i.e. "mood swings" and often normal mood in between. ADHD is a condition that is not episodic.. Patients with ADHD have a neurologic disorder which can cause psychiatric symptoms but symptoms are related to impairment of level of functioning which is due to areas of brain involved in impulse control, motivation, and working memory not functioning normally due to genetically induced lack of normal or full development.

I was wondering if you might be able to direct me to some good resources that help verify those distinctions as I have a good friend who I suspect may have issues with adult ADHD, but it's complicated by the fact that he's had two reactive cases of major depression, the first being unipolar, and the second what looked like a breakthrough bipolar 1 episode. Dr. Applebaum was very informative especially regarding drugs for Bi-polar patients. Thank you for the information. I have been taking lithium citrate for about 30 years and now in Canada it is being discontinued. I find this VERY upsetting because I cannot take the lithium tablets. I drink much too much liquid with the tablets. Any thoughts on what I should do ? Marilyn
I was misdiagnosed for years ever doctor just threw medication at me. I had some doctors either think it was manic/ clinical/ personality. Just recently got on with ssi found out because i was not properly treated my bi polar got worse and extended into a cardiovascular disease. I cant even afford the meds that did help 4k a month just no way
Plant Derived Colloidal Minerals containing natural lithium is all I've ever needed to take care of, what was, severe Bipolar Disorder. I just add it to all the water I drink and to the ice cubes. I would like to read the details of a controlled expreiment involving hask and marijuana in its natural state and what effects these drugs have on different patients. so far all i have seen is atest where only about 20-30% of subjects missused marijuana, the rest missused alkohol, crack, cocain, pills amf, ect.. Rule of 3? I would never get diagnosed if the anti-d's didn't land me in the hospital. That's a terribly difficult threshold- 3 devastating events. Some of us are strong despite the illness. My doc said "you can't have that, you haven't crashed your life". I've had it for 40 years. You adapt. He was so sure they had me take failing ned after failing med that sent me to the hospital. Bad f'ing advice rule of 3. Stop hurting people.

cause I was so broken and disoriented finally I hit rock bottom and was standing on my balcony railing half a bottle of tequila to my dome when I don't even drink ready to jump off the 39th floor anyways long story short I managed to overcome this shit show and really started doing research on bipolar disorder

Funny how this guy assumes that the root of the problem is to give pills to the depressed person. Never once did he acknowledge some sociopathic boss, or parent is mentally abusing the person. I would encourage viewers to search for bullying in the workplace. This guy should get a real job, and have real world experiences with sociopathic bosses. About my future will this evil illness ever go away it seems not I am in despair anyone out there' who gone through and now flully recovered
When I was younger I was on Mellaril 25mg 4x daily and tegretol 100mg 4x daily. All I can say is the meds where crazier than the illness.
