When she Awakened and used Ochako's power, Toga was gleefully living and loving in the only way she knew how. Now, however, her hatred and sorrow are stronger than her love for Jin. One of the clues in the last episode was that the Twice clones were saying "Die, heroes!" in voices of utter despair and loathing. She's literally an emotional and mental mess, and it's affecting her ability.

If I'm not wrong, AFO will cease to exist due to rewind quirk. That's why AFO is in a hurry to reach Shigaraki.
The anime did say the more Damage he takes, the more the rewind quirk is used.
