The domino theory is way way out of date. It has been modernised. google 'bird and Germain domino'. the original blames the worker...the newer version puts emphasis on the employer and the safety system.'
most awkward example of for human factor theory....first u said overload was basically related to safety or ergonomic hazard. you also said a random noise would be associated with an inappropriate resonse. Then on the next slide overload was a result of a random noise and other psychosocial factors.....This kinda thing is what messes ppl up on tests
