    Sir, My mother is 66 years of age and has many gall stones hence required a gall bladder removal surgery(Lap Cholecystectomy). As she had a cardiac history of PTCA(Angioplasty) 5 years back so Surgeon suggested to get cardiac clearance first. When we met Cardiology doctor, he again found blockage in another artery of heart during Angiography and performed Angioplasty last month(15th July 2024). After 2 days of PTCA, she again had severe stomach pain and MRCP reflected that few stones have slipped to bile duct. Gastro doctor mentioned that PTCA has been performed 2 days back only so no surgery can be performed so he did not clear stones from bile duct and only placed a stent into CBD--Bilary Stenting. But next day Pancreatitis happened post ERCP and her Lipase and Amylase values shoot up. Lipase, Amaylase value's started coming down after 2 days and we got discharge after 2 weeks on 29th July 2024 with bearable stomach pain and slightly high values of Lipase and Amylase. Gastro doctor mentioned that these tests(Lipase, Amylase, LFT) will improve in upcoming 2 weeks and pain will also subsidize.
    Now, my mother again has severe stomach pain since 6 days along with fever and vomiting. PAIN KILLER injections are given since 2 days and that only works for 5-6 hours but pain re-occurs.
    Gastro doctor and surgeon says we need cardiac clearance before performing any surgery as patient is on dual anti-platelets(Ecospirin-75 and Ticaspan-90) and Ticaspan-90 needs to be stopped pre and post 5 days of surgery. Cardiac doctor says that no surgery should be performed before 6 months because if we stop any anti-platelet(Ticaspan-90) then heart stent can get blocked. He says that stopping Ticaspan-90(anti-platelet) and putting the patient on Clexane injection(anticoagulant) for 10 days also does not fully cover heart stent blockage.
    .
    We need your expertise to suggest:--
    (1) When can we get the surgeries performed(Gall bladder removal and ERCP(removing stent from bile duct and clearing bile duct)) as last PTCA is done one month back(15th July 2024)
    (2) Ultracet, MEFTAL Spas tablet, is not able to control pain so she has to take injections which subsidize pain for few hours but it re-occurs.
    (3) If surgeries can't be done in near future, how can we manage pain till then?
    Note: (i) Alkaline Phosphate, Lipase, Amylase are high.
    (ii) MRCP and USG whole abdomen performed today observations are as below:-
    OBSERVATIONS:-
    Gall bladder :- Over distended. Sludge is seen in the lumen with multiple tiny calculi. Diffuse edematous
    gallbladder wall thickening is seen, measuring upto 6.8mm with slight pericholecystic fat stranding. (USG
    correlation is suggested). Cystic duct:- Dilated. A calculus measuring 7mm is seen in distal part of cystic duct causing mild mass
    effect on adjacent part of CHD.
    IHBR:- Central IHBR are mildly dilated. Hepatic duct confluence is patent.
    CBD: Mildly dilated, measuring upto 7.4mm in diameter. Stent is seen in CBD. No definite CBD calculus is seen on present scan.
    Pancreas and main pancreatic duct :- Pancreas is bulky with slight peripancreatic fat stranding in head and tail region. MPD is not dilated.
    Other findings: Prominent periportal lymphnodes are seen.
    Comparison with previous MRCP dated 19.07.2024:- Stent is seen in CBD and mild acute pancreatitis.
    IMPRESSION- Acute cholecystitic with cholelithiasis. - Calculus in distal part of cystic duct causing mild mass effect on adjacent part of CHD.
    Mild acute pancreatitis.
    - Mildly dilated CBD with stent in it. No definite CBD calculus seen on present scan. Clinical and lab. data correlation is suggested.
