In Acol, the 2NT bid would just be a balanced 11-12 count without four hearts. It doesn't show three card support as the original bid could be a four card suit.
We haven't seen the Queen or Jack yet. If declarer had AQxxx they would usually just draw all the trumps. If they had AJxxx they would take a finesse. Seeing as they haven't done either of these it is highly likely they have 6.
Much prefer a low spade over and a spade back to the play of the Q, N might err after winning 2nd spade south can play any card safely .

can you explain why do we know that declarer has 6 trumps after the played the A then the 4?
