    Thank you for this video. It took me years to create a template that works for me (yet I still second guess myself). Watching this helped me realize it's ok to have an custom template that doesn't fit into one of the established formats.

    I use PIE notes which I find very useful. Problem, Intervention, & Effectiveness.

      This is great! I love hearing about a new template and this one sounds pretty efficient but like it also covers what's needed.

    I find myself making statements rather than always full sentences. ex Processed feelings of guilt related to parent's divorce. I like short and to the point. Does that ever pose a problem?

      Nope! In fact, brief statements listed more as bullet points are MUCH easier to read and review from an auditor's perspective :)

    I am subscribed and trying to locate one of your videos that I just watched yesterday along with multiple others. It was the video where you started out early in it by saying “there are five basic things that you need to include in your progress note.“ During the video you stressed about keeping the very sections of your note brief and to the point, and could even use listing or bullet points instead of complete sentences. If you can be of any help in identifying exactly which video that is. It would be much appreciated! Thought I saved it, but apparently I did not in the flurry of watching several, and really enjoyed them!

      @@MaelisaMcCaffrey yes, it is that video and we found it not too long after my last contact with you. I am already trying to use some of these strategies as they are great. It’s not so easy to break old habits but I’m making progress and this is definitely going to be better! I did have one question and that was are these type of notes also OK with Medicare?

      @@debbied1554 Yes, these are okay for Medicare but you want to be very specific about tying things back to the treatment goals and clearly identifying the ongoing impairments and progress.

      Even if you do SOAP notes, use checkboxes or bullet points to write the info for each of those sections! It'll save a lot of time and effort, and actually be easier to read the note later.
