Because people largely aren't familiar with the whole scope. And then, you would have to deal with the entire hierarchy, and traditions. And then you have to look at the differences of say French vs. English system. And we can get into hereditary title vs. non hereditary titles and...
But the real irony is: Once she becomes empress - the power that she would wield is immense, and the retribution on those that treated her poorly could be insanely severe. And if the husband is in poor health but not liable to die - a strong trust forming between Empress and Emperor would defacto leave her as the master of the entire Empire. THEY WOULD ALL KNOW THIS.
Honestly, some minor fixes to the plot line, and how the marriage came to be would resolve everything. Such as the Emperor insisting on this match himself - and demanding it for the perception that, as the illegitimate child, treated with mild disdain, she would have no strong loyalty to her family or other nobility.
So many minor things fix otherwise big problems.
