Medicare insurance Lcd and ncd k denial karte hai tabe as per Lcd policy Article I'd k isab se payable DX agar medical record me nahi hai to pay nahi karega bhai appeal kar k fayda nahi hai. Sirf commercial insurance Co-50 k denial karte hai tabe appeal kar k payment ata hai
