The scene made me gag. But it's basically a rape scenario turned the other way around, so I can see why the writers thought it was fitting karma for a sexual deviant. Maybe the scene will make more people realize that rape isn't erotic, it's torturous and disgusting.

A side effect of his exposure is that he breastfed up until an inappropriate age for kids to breastfeed, which is why he's called Mother's Milk. In the show, Homelander drinks Madelyn's breastmilk, probably due to his neglect as a child, but I think it's a clever way to incorporate that part of the comics into the show, while also exploring Homelander's descent. I highly recommend the comics, but they are seriously darker than the show, which is hard to believe because the show is pretty gruesome. But serious content warning if you do choose to read the comics, it gets brutal.
During the gill scene, I laughed for a solid minute out of pure shock saying "no. What? No!" In between bouts of insane laughter and fighting back tears. Whomever wrote that scene is an evil genius.
