Every time I attach my rope grab, I give it a "yank" from the lanyard to double check its positioning. I actually double check all my devices on the rope prior to a descent...in the rare event I may have placed one upside down, this method guards against that.
You need to check for debris fouling or catching in the rope grab as you ascend. It can in certain circumstances completely stop the device from working.
Is it considered safe to use the rope grab on the same line as your descent device? I've always been told no...but rope is expensive. Yes, I'm aware that life is priceless.

You should have an independent safety and working line rather than using the grab and descender on the same line.
