Achievement/Trophy delays are sometimes the result of network connectivity. Reboot or sign out and back in for a fresh connection if you're certain you've performed the description sufficiently to unlock said achievement/trophy.

That was my experience as well. I tried with Georgia, just assuming the #1 team would make the playoffs. Glad this helped!
