I think the reason why Cartman sounds much more like a genuine blood thirsty psychopath than Sonic.EXE here is because Sonic.EXE’s character is supposed to be depicted a bit calmer but still a little eerie, while Cartman on the other hand is…. Eric Cartman.

I’d imagine this take place when Scott Tenorman accidentally ate his parents remains, with the large crowd watching from the foreground and the background, but instead of Scott, it’d be Boyfriend. After showing a cutscene of this, the song would start, showing Cartman directly on the table facing Boyfriend who would look distraught. After Cartman laughs at 0:45, The people watching would turn into corpses. After 2:25, I’d imagine large black liquid coming out of Cartman’s mouth, making his face even more distorted than it already once was.
