  • Suboxone (buprenorphine-naloxone) is a powerful tool to help treat opioid use disorder. When taken appropriately, it is life saving!
    That said, Suboxone isn't a cure for addiction in itself and it has side effects as well. Therefore, Suboxone should always be prescribed in the context of a comprehensive treatment plan.
    In this video, Dr McGeachy explains the seven things he recommends to all patients taking Suboxone. (Everything in this video could also apply to patients taking Subutex or methadone, for that matter.)
    We aren't going to spoil the video for you, but you can probably guess two of them based on the thumbnail. Can you guess any of the other five? Leave your predictions in the comments below (before you watch the video, otherwise, what's the fun in that?). Leave suggestions for new videos in the comments below or share your Suboxone treatment tips. No questions pertaining to personal medical questions.

    I wish any one of the 4 different doctors I’ve had prescribe me suboxone mentioned even a single one of these tips. Right now, I’m getting it prescribed from an online only doctor because every one of the “clinics” I’ve been to have shut down with next to no notice. It’s exhausting going through the intake process over and over and it seems like Suboxone is just a hustle to make a profit off of keeping people dependent for the docs I’ve had. I’m glad there’s still people out there that care. Thanks for your help doc.

      I am ambivalent about those online Suboxone services. It is good that they expand access, but they don't offer much besides a prescription.

    As well as harm reduction/ safe injection site services, Toronto pharmacies (large chains i.e. Shoppers Drug Mart/Rexall/ Guardian) will give Naloxone Kits no charge. This policy is probably the same in large urban centres nationwide.

      Thats not true for me. I don't get high on them but they do make me tired. I feel the tired coming on soon after i take the strip. The very first time i took Suboxone, i got about half whacked and thought i wasn't going to be able to take them but it never happened again going forward.

    I get pill splitters and pill organizers at the dollar store and try and make sure my pts have one of each. Don't know how you keep track of what pills you have taken without the organizer and the splitter really helps for people not on the film. I also recommend silica packets to keep in the pill bottle so they don't crumble when you split them.

      I've heard that from many patients; seems like increased bowel activity is a long lasting withdrawal symptom. People seem to experience out-and-out diarrhea for a limited time period but will have multiple bowel movements a day for months afterward.

      Addiction medicine is getting so complex. Many doctors don't have the time or energy to keep up with advances and new developments. This is especially true if their specialty doesn't include a lot of patients with pain or addiction.

    Also Doc...you should research what long term Suboxone is doing to people's teeth. I'm not the only one. Always rinse with water after to prevent tooth decay..

    • @JackMcG_MD
      @JackMcG_MD  Рік тому +2

      Thanks sharing your thoughts. Chronic opioid use, Suboxone included, can cause dental decay by reducing the production of saliva. I also suspect there is an aspect of "catch-up" which occurs when a patient is stabilized on Suboxone. By which I mean, they start taking care of themselves after a long period of neglect and discover the dental decay which occurred while they were using.

    i started taking suboxone about 15 years ago on a medical study at a methadone clinic, they tapered me down 1 mg a day for 16 days. which hurt. but the big hurt was the difference between 1mg and 0mg. ASTRONOMICAL difference. so i was forced to find other means. i've been through 8 or 10 doctors in the destitute medical system and the pharmacies are more of an obstacle than anything else. ive tried multiple times to ween off the drug, but the DTs never let up. they just dont quit, and of course its taken nearly all my teeth. encouraging story, right?

      Ugh I was on 110 mg methadone and got off of that through a slow tapper but fell off of the wagon and was introduced to sub as a substitute to methadone without all the side effects and was told when I was done using it I could hop off with no withdrawals! Well that was a huge lie for me at least! I'm down to 2-4 mg a day and some days a little more than that but absolutely no more than 8 mg per day. I've come off of it before relatively easily but this time has been different. My goal is 2 mg a day and stay on that till I fully adjust before dropping again!

    • @innerunderbelly
      @innerunderbelly Місяць тому

      @@whocares4464 i have an extremely similar story... i tapered off methadone verrry uncomfortably, and i was put in a medical study on suboxone with the same clinic, i was told all the same lies... the difference between 1mg and 0mg is ASTRONOMICAL. im stuck on 4mg, yep.
      Addendum: lol i forgot that this was literally the original comment already

      Butrans is the brand name in the United States for transdermal buprenorphine and it is only indicated for treatment of chronic pain. That's a shame because that makes it illegal to prescribe it for the treatment of opioid use disorder. If not for that fact, Butrans could be a useful tool for buprenorphine induction while avoiding PW.

  • @USViper
    @USViper 10 місяців тому +1

    Number 1. Quit suboxone. Its just another opioid. Methadone and suboxone are just crutches. Im off opiates and now off opioids (suboxone) my brain rewired itself back to normal, and I love life again.

    • @JackMcG_MD
      @JackMcG_MD  9 місяців тому +4

      I'm glad you were able to quit and have a successful recovery! You've accomplished something that few people are able to do successfully.
      That doesn't mean that Suboxone isn't effective for other people, however. Calling it a crutch ignores that fact that this medication saves lives and changes them for the better. I know many patients who would not be alive now without Suboxone. For them, it's an essential medical treatment.
      Also, what's wrong with a crutch? People with disabilities need assistive aids to navigate their world. Crutches, walkers, and wheelchairs all help people live their best lives. Would you tell an amputee to stop using their prosthetic leg?
      The only things that matter are keeping people alive and enabling them to live their life to the fullest. Our best scientific evidence tells us that Suboxone does just that and I am an advocate for that reason.
      What helped you in recovery?

    • @JackMcG_MD
      @JackMcG_MD  Рік тому +4

      You make a good point that Suboxone is not only used for addiction treatment, but also for pain management. Suboxone (buprenorphine-naloxone) only has a clinical indication from the FDA for treatment of Opioid Use Disorder, but it is used off-label for chronic pain too (the only buprenorphine containing drug with an FDA clinical indication for chronic pain is Butrans--the transdermal patch). This video is largely intended for people using Suboxone (buprenorphine) for treatment of opioid use disorder.
      A better title for the video would have been "Seven Things that Every Person Taking Suboxone for the Treatment of Opioid Use Disorder Needs" but that's just too long for a UA-cam video. That said, a lot of the items in the list could benefit any person, particularly in regard to the use of Miralax for management of constipation.
      Thanks for your comment!

      The thing is that even people taking legally prescribed opiates are still addicts. Being that it just means that you are addicted. The problem isn't addiction itself,the real problem is abuse. Anyone who is on these meds long-term is physically and mentally addicted. Some people do can maintain the regimen that doctor prescribes 4 years and years. And it helps them live a better life. Whether the problem is chronic pain or it's taken for maintenance, it's a slippery slope and very easily to slip into a habit of abuse.
