It mainly depends on your design code that you are using. ASCE 41-17 recommends fundamental mode load pattern for nonlinear static procedures.
How can we calculate the monitored displacement in sap2000 or etabs in the pushover analysis
You have to assign a particular value of displacement at a node. The displacement of this node is called monitored displacement.

why you consider 2 modes( mode 1 & 4)) in pushover load case?
It was only for illustration purposes. You can any mode for doing pushover but generally, the fundamental mode is used for buildings not having higher modes effect.
