I know of a senior, experienced ruqyah shaykh in SAudi arabia and I know many people whom Allah have cured through his hands. Alhumdulillah. (Note that he reads quran purely, and azkar from hadeeth- no hanky panky stuff alhumdulillah) and he said it DOES matter and the patient might not hearing even though he listens so it SHOULD be with headphones as opposed to speakers.
