How Long For TRT to Work?

  • In this video we will address the question of how long for TRT to work. Many of my testosterone replacement therapy patients ask similar questions about how long it is going to take before I start to see the benefits of testosterone injections or topical testosterone. How long for testosterone therapy results. The answer really depends on a few different things. I will say right out of the gate that initially it is going to take about three weeks before you notice the benefits. However you may not get the full benefits for almost three months. In between that time there's a few different things that you want to make sure are optimized and a few different things that can accelerate the benefits. That's what most of this article is going to be about. Many people starting testosterone replacement therapy wonder how long for their TRT will start to work. On average I expect people to feel some improvements in about three weeks. For some it can take longer three to six months even. In the case you start feeling better early on you may not get the full benefits until several months down the road. Firsts let's look at what symptom improvements you are expecting to see on TRT or testosterone replacement therapy. The main benefits I expect to see when someone has low testosterone and they start testosterone replacement therapy are energy drive and mood. Along with this you will also typically see improvements in erectile dysfunction and libido.

    The benefit that most people notice first is with energy mood and drive. How long depends on:If your dose is correct, If your estrogen levels are not going up, What's happening with your androgen receptors.
    These three things are important but I also wanted to point out that most of us think a little bit is good so more is even better. That's not always the case for testosterone replacement therapy. The reason it is not is tied in with the downstream hormone metabolites of testosterone and other hormones. If you're on a high dose of testosterone and your levels are really high, your enzymes will be saturated. The are enzymes like alpha five reductase and aromatase. The alpha reductase turns testosterone into dihydrotestosterone (DHT) and the aromatase makes estrogen or estradiol. The more testosterone in your system the more DHT and estradiol. These can cause some side effects. In the case of estradiol, it can diminish the positive effects of the testosterone. So if you're getting a lot of estrogen production with the increased testosterone it may negate some of the benefits . Sometimes you just decrease the dose testosterone and things even out. Most people know that high estrogen negate some of the benefits of their testosterone replacement therapy. What they sometimes forget is that, you have to check the levels at different times. You have to make sure you check it later on (maybe 6 mo) into the treatment. Once you get the estradiol optimized, you will see more benefits but the high estradiol will delay the time it takes you to see benefits. That doesn't necessarily mean you have to discount the total time and start back from square one. it may take an additional 3 weeks after the estradiol is normalized.

    Feels better incrementally in about 4-5 days. If your levels are super low, you'll start to feel it faster. If you go to the gym and exercise, you'll start getting stronger within a few days of taking it.

      I’m on week 3 and felt it after 5 days, less lag throughout the day. My shit was 98 at 3pm and 240 10am. Def raised it quickly

      A few days? Not the case for vast majority of people…guys shouldn’t have unrealistic expectations…if you felt better in a few days, I’d attribute it to the placebo effect

    I was around 240 test level. Hadn’t worked out regularly for about a year. Started Androgel and went to the gym and 200 lbs on the flat bench felt so easy even though I had gotten skinny. But the first test cyp gave me an instant rush. Felt so weird but good!

      That is what’s crazy. I do lift regularly and train jiu jitsu. My diet could be better, but I am not low in vitamin D either. My testosterone levels were at 232. I got prescribed TRT cream recently. On day 3…dont feel anything yet

    was very deep in testosterone. Felt better after second day of TRT with Testogel 1 pump per day. My insuline came also down quickly - nearly on normal insuline levels.

    I have been struggling really badly with cortisol on all doses and had my levels tested throughout the process for 2 months.
    The point you make about testosterone receptors not binding as well as dht due to potentially not doing much exercise is very interesting and might be my last hope.
    My question is, if the replacement therapy is binding to dht and not testosterone because I don’t have enough receptors would that cause high cortisol? Because I hadn’t trained well for years before starting TRT due to injuries

      Basically no but exercise increases androgen receptors and testosterone and DHT bind there.

    I'm been on Androgel ( *2 pumps of 16.2 mg per gram pump each morning ) for 6 weeks I don't feel any better, I'm still tired and slightly dizzy all the time with low drive, no improvement in muscle development or exercise performance and still have joint pain.
    So for me it's not worth the cost ( $60 per 44 days ).
    My T levels were in the low 200s so I'm getting them retested to see if they went up as a confirmation.

    Hey Doc, I really appreciate your videos and wealth of knowledge. I’m on my second week of TRT and I just can’t sleep. Falling asleep is a non issue, but I’m wide awake by 1am with surges of energy. Is this a dosage related issue or a reaction to the medication itself?

      When you say “I’m on TRT“ would this also happen to include estrogen control with AI pills? If so, get off the stupid AI. The doctors putting their patients on aromatase inhibitors are out of their minds. They have no idea what they’re doing. Blood levels of E2 are virtually meaningless. These practitioners doing E2 control simply haven’t studied it.

    It takes far less time if you're already in excellent shape from weight training, eating right and proper sleep. .

      Possibly but for everyone there is a time interval that it takes to reach full tissue saturation. If you are exercising you do have more androgen receptors and we would expect this to speed things up.

      What does eating right mean though? I start try tomorrow and have been losing weight over the past 6 months. Thing is I’ve been taking a more carb heavy approach. A lot of rice potatoes oatmeal etc. I’ve lost 36 kg (with riding my cycle a lot) do I continue with my current diet as it’s worked for me so far ?

    Is there a benefit to testing free DHT levels or should you really just be concerned with total? Would injection frequency changes, keeping dosage the same, have an effect on DHT levels?

      don't thins there is much benefit to free dht if you know the free testosterone but it would depend on the situation. increased frequency and smaller dose will /should reduce dht.

    At age 25, I didn't notice one single benefit from bringing a 190-220 ng/dL TT level, tested 3x, up past 700 ng/dL with FT very high (29-37 pg/mL.) Zero libido, no ability whatsoever to put on muscle mass with a hypercaloric diet and resistance training. Also, I was underweight, not overweight at the time (due to serious lack of muscle.) TRT just simply... didn't work at all. 100% hype.

      You're not the first person who has to deal with such anomalies that these "expert" doctors can't explain.
      For me personally, I have to be overweight to have normal testosterone levels. If I lose bodyfat, it causes my testosterone to crash and I discovered this the hard way.

    Hi Doc. Been on Test cypionate for 6 months. Good outcome with 850-900 total test and high free test but I feel like I may be allergic to the carrier oil as I get some itching and irritation. I inject sub q with insulin needles 3 times a week. Would enanthate be a good alternative?

    Please READ DOC I've been wanting to take test for a while I'm 57 but my family doctor said my levels wasn't low enough. i also go to a back doctor for back pain "arthritis "take pain meds and I'm still sore every day from working i mean im miserable he suggested that I inject test said it greatly improve my quality of life and I'm like hell yes I'll do it so i just started and hope this will give me more energy and maybe lose a few extra pounds wish me luck. Also I take 1ml every 14 days is that not enough or is it ?can you increase the dosage ?if im not noticing any difference.

      It’s probably enough, but you need to be injecting more often than 14 days you should split it up to twice a week

    Great information. I’m thinking about starting testosterone… however my number 300s but my doc don’t think I need it. Systems I have is very low sex drive, penis flaccid, no energy to go anywhere.. however I do workout 3/4 times a week and my age is 56. There’s more but that’s enough. What’s your thoughts? Thanks

      Based on your symptoms, you appear to be exhibiting the consequences of low testosterone. Numbers are not everything.
      What advice did your doctor give you based on your symptoms?
      Are you doing anything to help boost your natural testosterone production or further investigations to determine the cause of your low T ? Like your LH FSH levels?
      About 3 months ago my natural testosterone production crashed and went below normal. I discovered that if my body fat drops below a certain level, it causes my testosterone production to crash. No doctor can explain why. When I gained weight, my testosterone levels recovered.
      Make sure your doctor does all the necessary investigations to help determine your low T, as maybe it's something you can fix without resorting to TRT.

      Your looking for a ratio? No just want to avoid high estradiol and low estradiol. Ideal is about 20-35 pg/ml.

      Estrogen naturally goes up linearly with testosterone but either way u want to control how high it goes and it’s beneficial to have some estrogen

      @@swintegrative thank you so much Doctor. I read some studies and was unable to find a reliable source of information based on the subject matter. When you say changing, do you mean the dosage or different meds. A car rear ended me and left me whiplash and hurting. I notice that my trt treatment has been not as effective as lately. Honestly speaking, I never had a surge of energy described by some people on the comments and my body really has not changed since within 4 months on trt. I don't know what to expect.

      Twice a week is now the most widely accepted starting point. Anybody starting out on once a week or less frequent have no idea what they’re doing.

      Its more than just weight. Skin permeability is also a factor. You don't know these these thing until after your blood work. But you can certainly tell if you feel improvements.

    Hey Doc, I have low testosterone level 300 Ng and my age is 32. I started 250mg enathate four weeks ago but I don't see any difference. My sex drive is still very low. When should I expect a change in my sex drive.

      The “cancelling out” effect is nonsense and doesn’t happen unless you are taking very high doses and your talking about mental effects. Anxiety, feeling uneasy etc…

    hey doc, I have started TRT 3 months in. My T levels are 600ng/dl at E2 at 58 pg/ml and my morning cortisol at 24ug/dl. I have yet to see any benefits of TRT. My protocol is 25mg Test Enanthate Every other day Subq. I am considering aromasin as AI for myself. It would be great if you could suggest the dosage of AI?

      Hi Ateeb, thanks for the question. I can say that a cortisol of 24 ug/dl and estradiol of 58 pg/ml seem high and this could be why you have not noticed much. Sorry, I can't give you specific dosing advice in this format.

      Your dose is too low. Stop worrying about estradiol. It’s not that high. Does anybody go around checking the estrogen levels on healthy 18 year old males who have total T levels over 1000? No, they don’t, and neither should you worry about yours.

      The vast majority of people get improvement from it but like with anything it depends…what r u using it for Getting the dosage right and carful monitoring…All the doctor stuff. Problem is not every clinic does it the same and that is where is get mixed results.

    Just did my first blood hormone panel ever. Is this a good blood panel need your input? I have all the symptoms of low T except for ed, I'm 5ft9" just cut from 200lbs to 187lbs train 5 days a week volume and intensity heavy too crazy strong physically. But feel like crap mentally

    Need your advice brother. I'm 35 years old lifelong Natty. I normally transform my body way faster than the average person with proper food and decent sleep training hard of course. However when I stop training and eat like trash I also get out of shape just as fast. So i can work out for 12-20 weeks 5 days minimum a week and literally look like I been training for year or years. I been working overnights for the last year and not sleeping well I been training again from taking 5 months off got out of shape and now am some months into training now but this year has been weird I'm not transforming as fast. I just had my hormone panel blood test done for the first time in life would like your input on it.

      Poor seep can really affect recovery and hormone output. Your levels look average not low not high. not optimal but not super low. Sleep would be my first guess.
