    I'm on Lantus at night and Januvia at breakfast. My morning glucose reading is fine, it is 2 hours after I eat I'm anywhere from 200 to 300 no matter how small the meal is. My sugar does go down but it takes 4 or 5 hours to get it to say 120. This is virtually with every meal unless all I ate was a sugar free jello. So as far as I'm concerned my meds are not working,as far as meal time control goes. I have asked to see an Endo and it falls on deaf ears. I have been a Diabetic for almost 40 years, so I'm pretty knowledgeable on taking care of myself. I don't know if its an insurance issue or a doctor issue, but the doctor seems out of it as far as treating me. I'm rushed through appts and I can tell the doctor is doing nothing by playing with her computer. . My A1c usually was around 6.8 now its 7.8, and that doctor is fine with it. I'm not fine with it. It is very difficult to change doctors as it means a waiting list for another one. What to do I don't know.

    I tried to chase my morning insulin with lantus and no matter how much i took, it kept hitting 200-220. I was on 54 units before i decided to work my way back to 10, and it only required an increase of 5 units bolus insulin per meal. Taking more lantus caused my insulin resistance to greatly increase and my doctor told me i am the most insulin resistant patient she ever had. My a1c is higher now, but i am trying to improve it through dietary changes because going back to 54 units lantus means i would need 10 units bolus just to eat what amounts to lettuce and vinegar. I am only eating 100 carbs per day and was taking around 100 units total.

    Thank you so much for providing precisely the information I was looking for. I'm currently on GLP-1, and my fasting blood sugars are at 150. I'm grateful for this insight, and I'll be discussing the possibility of long-acting insulins with my doctor.

    Well that’s interesting I’ve been on Levemir for 6 years and I always thought it was 24 hour coverage. I might consider switching to treesimir now as I’m trying to curb morning highs. Thanks for the info 🙏

      It can be that the blood sugar levels are lower than before-- your body is adjusting. How are you feeling now?

    I used to be on ABASAGLAR( start with 10 units) and it's never worked on me so I ended up going really high dose and inthe end I was put on OZEMPIC.
    I got fed up with the weight gain and hyperglycemia all the time so I tried to go on KETO for 3mnth and I completely off all my Diabetics medications. However, my Dr. thinks that I should go on metformin just for preventative since Diabets are in the Genes. What do yiu think of their descission Dr. Diana?

      Hi Sylvia! So impressive. Keep up the great work. although I cannot give advice, metformin is a great medication that we use for prediabtes and diabetes. If your blood sugar levels are borderline, normally we can consider metformin to help keep sugar levels in the more normal range. Diet and exercise are always first-line... metformin can be used if needed if those are not enough to maintain good sugar levels. Hope this helps. Best of luck :)

    hello do you know what’s the chemical difference between long and fast acting insulin? like why exactly does long acting last for like 24 hours? does it have membranes on the insulin or something?

    Thank you, Diana, so so so much for your wonderful content. I learnt a lot of from you!
    A question, if I may: my mom injects her 9 units of Tresiba at 12 am every day, and 10 units of Novorapid for breakfast at 10 am, has a low-carb snack around 2pm (no Novorapid there), 5 units for lunch at 5pm, and 5 units for dinner at 10 pm.
    I have observed that her glucose levels get dangerously low around 12pm or 1pm, and again at about 7pm or 8pm, especially if she's been particularly active that day, or didn't feel like eating her typical meal portions. Any thoughts what might be happening and how to fix that?

      Hello, so sorry to hear that. Her insulin has to be adjusted unfortunately. She needs to discuss that with her doctor.

    Due to my new diet I am no longer having any spikes and now use < 5 units of Novalog to cover a meal (sometimes). Should I stop Triseba? I don’t see the need for it anymore.

    So if tresiba stays in your system for more then 24 hours and you take it everyday how does that not cause you to have low blood sugars?

      If can be-- therefore people can be flexible, especially those who may not wake up at the same time every day.... Its a must longer duration than your older ones. I hope this helps and that you are doing well :)

    Dang, I take 50 units of Levemir in the morning and 70 units at night and my fasting BS are still 180

      Yes, generic and much cheaper. I do prefer Tresiba and Toujeo however, we have to be realistic and what will be the most cost effective for the patient.

    I take two shots of Lantis per day I take 13 units in the morning and I take 10 to 12 units before bedtime

    I took 15 Tresiba units and now I got Toujeo or should I also take 15 units what is the difference

      Hi Michel, sorry for the delay. They are usually 1 to 1-- so if you take 15 units of Tresiba-- you would take 15 of Toujeo-- they are essentially the same medication. Both very great.

    Insulin makes fat period, all insulins raise my sugar, doctors dont know whether Im type 1 0r 2. One swears Im 1 another tested me said no Im not. All I know is I feel like crap the more I take isulin. The liver is the problem in all diebetics, insulin makes fat, not good!😮

      hmm, how is that possible? Its very easy to test. Are you seeing an endo? If not, that would be my suggestion.

    I know this video is a bit old but its like no matter how much humalog i take lets say for instance 1 unit every 8 carbohydrates or 1 unit every 10 carbohydrates its still running in the high 300s with 1 unit every 10 carbohydrates with 63 carbohydrates when i eat .But with 1 unit every 10 carbohydrates its running in the high 400s and im taking 20 units of Lantus everyday 24 hours recommend. This just totally happened to me recent in the last week like my body changed or something i have no idea .So that would mean they need to increase my Lantus ?And i started taking 1 unit every 40 past 140 lastnight and it still running 316 and i took humalog and ate at 1245 this morning like 5 hours ago now its 602 am just wondering if my Lantus needs increased i beleive it does decrease of lantus means to help my sugar stay up but increase will help it stay down right ?

      Tresiba can replace Basaglar. Usually patient's take one or the other. Not both.

      Please, whatever you do, don't give up! There are so many different treatment options out there. Talk to your health care provider about experimenting with newer medications and different diets. If they don't cooperate, seek another medical opinion, and another, and another! It's your right to receive proper medical advice! We've come so far in the field of medicine and especially diabetes! Never, ever give up 🙏

    All I have to say is the medical people that make all this stuff is making millions of dollars of insulin and diabetics what and where did it come Simpson everybody’s a diabetic now I’m 66 years old. I am now the stage two diabetic crazy I’m not overweight nothing but here we are.

    Useless. Nothing of interest here, just basic info that everyone should know if they are taking long acting insulin. I love the uniform she's wearing, like a Doctor about to do surgery....... Presumably to tell us she really really really is a doctor (is she?).
